Aim: To investigate the relationship between caries etiological factors and occurrence of severe caries among adults in Kuwait. Methods: Forty-two adult patients with severe caries, attending Kuwait polyclinics, were compared with 36 caries-free subjects in a cross-sectional study. The patients with severe caries were aged 16 years or older and had a minimum of eight open coronal carious cavities, with the involvement of at least one anterior tooth. In addition, their salivary glands had not been compromised by disease or medication. The caries-free subjects had at least 24 standing teeth, and no carious cavities. Salivary flow rates, buffering capacity, frequency of sugar consumption, and oral hygiene index were measured. Mutans streptococci and Lactobacillus counts were determined by the dip-slide method. Results: Fisher’s exact test or χ² test showed that the patients with severe caries had a significantly higher frequency of sugar consumption, plaque index, Lactobacillus and mutans streptococci counts, as compared with those who were caries-free. In contrast, no significant difference was observed in salivary flow rates or buffering capacity between the two groups of patients. In the multiple logistic regression analysis, only frequency of sugar consumption, oral hygiene, and mutans streptococci count were found to be significantly associated with the occurrence of severe caries. Conclusion: Severe caries may occur in Kuwait even among adults whose salivary gland functions have not been compromised.

RESEARCH dental health educationAim To determine the effect of dental health education (DHE) on caries incidence in infants, through regular home visits by trained DH Educators over a period of 3 years. Method A randomly selected cohort of 228 children born between 1st January and 30th September 1995, in a low socioeconomic/high caries suburb of Leeds (UK) were divided into the following groups: A) DHE focused on diet; B) DHE focused on oral hygiene instruction (OHI) using fluoride toothpaste; and C) DHE by a combined diet and OHI message. DHE was given using an interview and counselling for at least 15 minutes at home every 3 months for the first 2 years and twice a year in the third year of the study. A fourth group D was given diet and OHI, at home, once a year only. All children and mothers were examined for caries and oral hygiene . A fifth group E (control) received no DHE and were never visited but examined at 3 years of age only. Results In the groups of children visited regularly only two developed caries and three had gingivitis (all in group A). In group E, however, 33 % of children had caries and nine (16%) had gingivitis. The differences in caries levels and caries risk factors between study and control groups were statistically significant (P < 0.001). Mothers of the study groups also showed an improvement in their own levels of gingivitis, debris and calculus scores by the second and third examinations (P < 0.001). Conclusion Regular home visits to mothers with infants, commencing at or soon after the time of the eruption of the first deciduous teeth, was shown to be effective in preventing the occurrence of nursing caries.

Foi avaliada a prevalência de cárie na dentição decídua de crianças entre 0 e 6 anos, matriculadas em creches dos Municípios de Bauru e São Paulo, SP (Brasil). O primeiro grupo (Bauru) não recebia cuidados sistematizados de saúde na instituição e o segundo (São Paulo) apresentava uma rotina de cuidados como norma institucional. Foram analisadas as variáveis relativas aos modos de viver desses grupos populacionais e sua associação com a ocorrência de cárie, efetuando um estudo de caso para caracterização de fatores coletivos de risco à cárie. Através de análise de regressão múltipla, verificou-se a influência da idade e freqüência de consultas odontológicas sobre a prevalência de cárie na amostra estudada (p<0,05). Na faixa etária de 5-6 anos, 23,3% das crianças de Bauru e 9,3% de São Paulo estavam isentas de cárie, contra a expectativa de 50% prevista na Meta n.o 1 da Organização Mundial da Saúde para o ano 2000. A prevalência de cárie foi mais elevada em Bauru nas crianças de 3-4 e 5-6 anos, apresentando significância estatística apenas para o grupo 3-4 anos (p<0,05). Não foram observadas diferenças estatisticamente significantes entre os sexos quanto à ocorrência de cárie.
