Diabetes mellitus may lead to intervertebral disc degeneration (IVDD). Matrix metalloproteinase‐13 (MMP‐13) is one of the major catabolic factors in extracellular matrix (ECM) metabolism of nucleus pulposus cells (NPCs) and contributes to diabetic IVDD. Bromodomain‐containing protein 4 (BRD4) is a member of the bromodomain and extraterminal protein family and is implicated in chronic inflammation. Here, we report that the expression of BRD4 and MMP‐13 was elevated in diabetic nucleus pulposus tissues as well as in advanced glycation end products (AGEs)‐treated NPCs; also, the regulatory effect of BRD4 on MMP‐13 was studied. We found that MMP‐13 was regulated by MAPK and NF‐κB signaling as well as autophagy in AGEs‐treated NPCs. Next, we explored the role of BRD4 in regulation of MAPK, NF‐κB signaling, and autophagy. The results showed that BRD4 is the upstream regulator of all of these 3 factors, and inhibition of BRD4 may suppress MAPK and NF‐κB signaling while activating autophagy in AGEs‐treated NPCs. Finally, we demonstrated that BRD4 inhibition may suppress MMP‐13 expression in diabetic NPCs in vitro as well as in vivo; meanwhile, it may preserve ECM in diabetic rats. Diabetes mellitus (DM) is a dependent risk factor in the progression of intervertebral disc degeneration (IVDD). High glucose supply has negative effects on nucleus pulpous (NP) cell and mesenchymal stem cell (MSC) biology. However, the effect of hyperglycaemia on the biological characterization of nucleus pulpous-derived mesenchymal stem cell (NPMSC) has not been investigated previously. Therefore, further exploration of the effects of DM-associated hyperglycaemia on NPMSC biology is important to better understand and develop endogenous repair strategies of DM patient-associated IVDD. Therefore, the cell biological characteristics were compared between NPMSC cultured in media with low glucose concentration (LG-NPMSC) and high glucose concentration (HG-NPMSC). The results demonstrated that HG-NPMSC showed significantly decreased cell proliferation, colony formation ability, migration and wound-healing capability compared with those of LG-NPMSC. HG-NPMSC also showed significantly decreased expressions of stemness genes and mRNA and protein expressions of silent information regulator protein 1 (SIRT1), SIRT6, hypoxia inducible factor-1α (HIF-1α) and glucose transporter 1 (GLUT-1), whereas increased cell apoptosis, cell senescence and caspase-3 expression. These results suggest that high glucose may decrease proliferation and stemness maintenance ability and increase apoptosis and senescence of NPMSC.Significance of the study: We found that high glucose concentration significantly decreased cell proliferation, colony formation ability, migration and wound-healing capability of nucleus pulposus-derived mesenchymal stem cells. Moreover, high glucose cultured nucleus pulposus-derived mesenchymal stem cells showed significantly decreased expression of stemness genes, related mRNA and protein, whereas increased cell apoptosis, cell senescence and expression of caspase-3. The present study indicated that better control of high concentration glucose in the early stage of diabetes mellitus should be recommended to prevent or limit intervertebral disc degeneration. Одной из наиболее важных проблем современной медицины является диагностика и лечение больных с коморбидной патологией. Ранее под коморбидностью понималось наличие у пациента отчетливой дополнительной клинической картины, которая уже существует или может появиться независимо от основного заболевания и всегда отличается от него [1]. В настоящее время этим термином обозначают наличие у одного больного двух и более хронических заболеваний, патогенетически связанных или совпадающих по времени возникновения вне зависимости от активности каждого из них.Выделяют несколько видов коморбидности:1. Причинная -связанная с поражением разных органов одним патологическим агентом (например, висцеропатия и полинейропатия при алкоголизме).2. Осложненная -являющаяся результатом основного заболевания (церебральная сосудистая катастрофа при гипертонической болезни).3. Ятрогенная -вызванная вынужденным негативным воздействием лечения на пациента, при условии заранее установленной опасности той или иной медицинской процедуры (остеопороз при приеме глюкокортикостероидов).4. Неуточненная -предполагающая общность механизмов развития заболеваний, но требующую проведения Коморбидность сахарного диабета 2-го типа и боли в спинеСахарный диабет (СД) и боль в спине при дорсопатиях широко распространены и часто развиваются одновременно. При СД 2-го типа (СД2) встречаемость боли в спине существенно возрастает. Выявлена ассоциация СД с тяжестью боли, частотой ее хронизации и рецидивирования, наиболее вероятным механизмом которой является поражение межпозвонковых дисков, опосредованное накоплением конечных продуктов избыточного гликозилирования (КПГ). При СД концентрация КПГ возрастает. КПГ инициируют эктопическую кальцификацию и снижение клеточной плотности в замыкательных пластинках, изменения структуры позвонков. Клетки пульпозных ядер КПГ начинают продуцировать провоспалительные цитокины и хемокины, запускающие процессы ангио-и нейрогенеза. Так формируется анатомический субстрат для возникновения дискогенной боли. Дорсопатии развиваются вследствие влияния иных этиологических факторов, но по аналогичным механизмам системного воспаления. СД и боль в спине являются коморбидной патологией с общими звеньями патогенеза. Для предотвращения развития дорсопатий при СД2 перспективно использование препаратов, снижающих интенсивность накопления КПГ, ингибиторов провоспалительных цитокинов, например симптом-модифицирующих препаратов замедленного действия.Ключевые слова: сахарный диабет 2-го типа, боль в спине, межпозвонковый диск, конечные продукты избыточного гликозилирования, провоспалительные цитокины, нейрогенез, ангиогенез, симптом-модифицирующие препараты замедленного действия.Diabetes mellitus (DM) and back pain are widespread and often develop simultaneously.
