The nerve of osteoarthritis pain

Abstract: Objective. Cumulative data suggest that central sensitization may contribute to pain in osteoarthritis (OA) and present with symptoms typically associated with neuropathic pain (NP). We evaluated the responses from focus group participants on the knee OA pain experience for pain descriptions that suggest NP. Methods. Focus group transcripts were analyzed by 2 independent assessors for unprompted use of pain descriptors that suggested NP. Items from validated NP symptom-based questionnaires were used to guide t… Show more

“…Clinical implications of these findings might be: (1) exercise and weight loss programs should be particularly targeted to women, who constitute the vast majority of the high leptin level group; and (2) prospective studies are warranted to test the clinical usefulness of serum leptin as a biomarker (as a proxy for intra-articular leptin level) to identify subgroups of patients, who would particularly benefit from exercise and weight loss programs, to detect the presence of neuropathic pain reported to be common in knee OA patients [48,49], as well as to predict persistence of pain after total joint arthroplasty.…”

“…Abnormal sensations such as allodynia or hyperalgesia may be observed. The presence of symptoms of NP has been shown in rheumatic diseases such as fibromyalgia (6,7), osteoarthritis (8,9), and rheumatoid arthritis (10,11).…”

“…OA symptoms vary considerably and patterns of nociception are distinguished differently by patients varying from dull aches to sharp stabbing pain. Neuropathic descriptors apply to a subset of OA patients indicating a role of sensitization in the underlying pathology [3].…”
