The Influence of the Thyroid on Adrenal Cortical Function

Abstract: The evidence is quite conclusive from animal studies that an excess of circulating thyroid hormone results in adrenal enlargement, and hypothyroidism produced by surgical thyroidectomy or by administration of antithyroid drugs results in adrenal atrophy. This subject has been extensively reviewed by Money (1). Studies of adrenal size in patients with thyroid disease coming to autopsy have yielded conflicting reports. Means (2) reported no change in adrenal size in hyperthyroidism, Marine (3) and LeCompte (4) r… Show more

“…Apart from data indicating accelerated disposal of cortisol in hyperthyroidism and increased number of cortisol secretory episodes (16,17), there are few and conflicting data regarding the impact of hyperthyroidism on adrenal functional reserve. The response to an acute challenge, such as that imposed by insulininduced hypoglycaemia, has been reported to be normal in some, or impaired in other, more severely thyrotoxic, patients (18,19).…”

“…These observations indicate that a degree of adrenocortical hyperactivity would be sustained in hyperthyroidism as a response to increased need. Apart from data indicating accelerated disposal of cortisol in hyperthyroidism and increased number of cortisol secretory episodes (6,14), there are few studies regarding the impact of hyperthyroidism on adrenal functional reserve, most of which are conflicting.…”

“…This could be explained by an increase in cortisol synthesis to accommodate for decreased half-life. In the setting of partial or complete adrenal insufficiency, however, the ability to increase the synthesis of cortisol would be impaired leading to a net loss of cortisol (9). In another study, long-term experimentally induced hyperthyroidism was found to be associated with diminished adrenal functional reserve (10), which could provide additional explanation for the risk of adrenal crisis in hyperthyroid patients.…”
