The implementation of a nutrition protocol in a surgical intensive care unit; a randomized controlled trial at a tertiary care hospital

Abstract: Background Malnutrition in critically ill patients is linked with significant mortality and morbidity. However, it remains controversial whether nutrition therapy protocols are effective in improving clinical outcomes. The present study aimed to evaluate the effectiveness of a surgical ICU nutrition protocol, and to compare the hospital mortality, hospital LOS, and ICU LOS of protocol and non-protocol groups. Methods A randomized controlled trial was conducted at the Surgical ICU, Siriraj Hospital. The nutriti… Show more

“…Auiwattanakul et al conducted a study on 1,686 surgical ICU patients in Thailand and reported that patients reached their calorie goal after 7–10 days [ 48 ]. Another study on 170 surgical ICU patients in Thailand found that less than 40% of the patients reached 60% of their target calories absorption by the fourth day in the ICU [ 49 ]. The tendency of developing feeding intolerance and decreased calorie absorption as a result of ICU stays is a common observation that may impair the patient outcome.…”

“…Although EN was performed in the ICU, more than 50% of the patients had to pause EN because of excessive GRV, diarrhea, aspiration, or repeated surgery or procedure, which limit use of EN in critically ill patients [13,14]. For better course of EN, every hospital should implement an EN protocol [15,16].…”
