This study examined the social network correlates of improvement in lower extremity mobility among respondents aged 65 and older from the longitudinal sample of the Survey of Health, Ageing and Retirement in Europe. The study focused on those who self-reported having difficulties with four lower extremity functions: (1) walking 100 m; (2) rising from a seated position; (3) climbing flights of steps; and (4) stooping, kneeling, or crouching. Multivariate logistic regression analyses revealed that social networks were variously associated with improvement in lower extremity function at the two-year follow-up. The findings suggest that under certain circumstances, a lack of social support in late life may actually promote mobility improvement. The research also shows that family networks are not always facilitative of mobility improvement. This is in contrast to previously published social network research positing that supportive relationships help foster health and buffer stressors in late life. Family caregivers and social services should keep this in mind when devising treatment plans upon discharge from the hospital and implementing care management plans for frail older persons in the community.

Background and Purpose-Studies conducted in Western countries have found a robust association between social support and cardiovascular outcomes (eg, prognosis after myocardial infarction and functional recovery after stroke). However, less is known about the influence of social support on the same outcomes among Asian populations. In this prospective cohort study, we sought to examine the impact of social support on the incidence and mortality of coronary heart disease and stroke among the Japanese. Methods-We examined prospectively the association between social support and risk of coronary heart disease and stroke incidence and mortality within a cohort of 44 152 Japanese men and women aged 40 to 69 years, free of previous diagnosis of cancer and cardiovascular disease. A total of 301 cases of newly diagnosed coronary heart disease, 1057 strokes, 191 coronary heart disease deaths, and 327 stroke deaths occurred between the baseline questionnaire (1993)(1994) and the end of follow-up in January 2004. Results-The multivariate hazard ratios and 95% CI for stroke mortality in the highest social support versus lowest social support group was 1.45 (1.00 to 2.10) overall, 1.59 (1.01 to 2.51) for men, and 1.25 (0.63 to 2.46) for women. Social support was not associated with stroke incidence or coronary heart disease incidence and mortality. Conclusions-Low social support was associated with higher risk of stroke mortality in men. However, social support was not associated with stroke incidence, suggesting social support may be more important in stroke prognosis than preventing incidence. (Stroke. 2008;39:768-775.)

ÖZETNöroloji kliniklerinin en sık karşılaşılan hastalığı olan serebrovasküler hastalıklar, yuksek morbidite ve mortalite kaynağı olması nedeniyle önem taşımaktadır (1). Önemli olduğu oranda, tedavi güçlükleri ve hastalığın ilerleyiş öngörüsünün net olmaması nedeniyle karmaşıktır (2). Serebrovasküler hastalıklar ile uğraşan hekimler, tedavilerine yön verebilmek, hastalığa ilişkin beklentileri belirlemek ve hastalığın gidişini pozitif yönde modifiye edebilmek için prognoz hakkında öngörü sahibi olmak isterler. Serebrovasküler hastalıkların prognozunu belirleyebilmek için tanımlanmış birçok değişken bulunmaktadır. Serebrovasküler hastalık prognozunu belirlemede hekime rehberlik edecek değişkenler; hastaya bağlı, hastalık kliniğine bağlı , tanısal yönteme ve tedavi yaklaşımına bağlı olan değişkenler olarak belirlenmiştir. Hastaya ait değişkenler içinde; yaş, cinsiyet, etnik özellikler, eğitim düzeyi, aile öyküsü, risk faktörleri (atriyal fibrilasyon , hipertansiyon, hiperglisemi, ateroskleroz, vaskülit, hiperfibrinojenemi gibi), aile ve sosyal çevrenin desteği, genetik etkenler ve biyomarkerlar vardır. As well as being important, they are also complex diseases since they are difficult to treat and the prevision of their progression is vague (2). Doctors who are dealing with cerebrovascular diseases need to anticipate the prognosis in order to direct the treatment and to modify the expectations of the patient's relatives. Inquiries and problems of the patients and their relatives can be guided through sound responses in the future. There are various variables defined to determine the prognosis of cerebrovascular diseases. The variables that will guide the doctor in the determination of the prognosis depending on the patients, the medical clinic, diagnostic methodology and treatment approach. The age, the gender, ethnicity, educational level, family history, elements of risk (atrial fibrillation, hypertension, hyperglycemia, atherosclerosis, vasculitis, fibrinogenemia etc.) of the patient, the family and social support, genetics and biomarkers are the variables depending on the patient. Characteristics of the lesion; localization and its extents are the variables depending on the diseas. Motor deficits, aphasia, anosognosia, dementia, urinary incontinence, presence of sleep disorder, the improvement rate of the symptoms are the variables of clinics of the stroke. Abstract: This study does not provide evidence for the efficacy of psychosocial intervention to improve functional recovery in stroke. Although PSI showed greater improvement than UC, the differences were not statistically significant.
