“…Significant research effort has focused on investigating the efficacy of the two-layer method (TLM), the present state of the art for pancreas preservation. [3][4][5][6][7][8] In the late 1990s, many centers reported improvements in islet isolation outcome using TLM for CP. These improvements were attributed in part to increases in tissue ATP associated with enhanced tissue oxygenation with the TLM compared to the previously used CP in University of Wisconsin (UW) solution alone.…”

“…noting that reports of isolated human islet cultured in vitro showed a similar 2-4 fold stimulation of insulin secretion when grown in high glucose conditions Matsumoto et al, 2004).…”

“…The genetic basis for the glucose responsiveness in engineered cells is the vector, in which the TGFα promoter that is upregulated in high glucose conditions leads to increased expression of the down stream gene (McClain et al, 1992;Matsumoto et al, 2004). Having determined that the cell lines produced insulin in response to glucose, we asked if production of mature insulin in the engineered cells was uniquely dependent on furin activity, which is produced in glucoseregulated manner and ultimately is responsible for production of mature insulin.…”

“…The two-layer preservation method was another factor favouring higher islet yields and better transplantation results. It has been demonstrated that the two-layer technique oxygenates and activates the metabolism of the pancreas, leading to resuscitation of pancreases with extended preservation time and/or ischaemically damaged tissue [11,24]. This might explain the higher islet counts and the improved success rate of transplantation compared with the UW preservation method, although the two-layer technique was performed in only eight cases and cold ischaemia times were uniformly short.…”
