Introduction: Preoperative measurements of apparent chord mu length above 0.6 mm have been associated with higher risks for photic phenomena after cataract surgery with multifocal intraocular lenses (MFIOLs). Methods: This retrospective study evaluated patients scheduled for elective cataract surgery at a single tertiary medical center between 2021 and 2022. Pupil diameter and apparent chord mu length were analyzed for eyes with biometry measurements from IOLMaster 700 (Carl Zeiss Meditec, AG) under photopic light conditions, before and after pharmacological pupil dilatation. Exclusion criteria were visual acuity worse than 20/100, prior intraocular surgery, refractive surgery, iris-related procedures, or pupil abnormalities affecting dilatation. Apparent chord mu lengths before and after pupil dilatation were compared. In addition, multivariate linear regression analysis, using a stepwise method, was conducted to assess possible predictors of apparent chord values. Results: Included were 87 eyes of 87 patients. Mean chord mu length increased after pupillary dilatation from 0.32 ± 0.17 mm to 0.41 ± 0.17 mm for right eyes (p < 0.001) and from 0.29 ± 0.16 mm to 0.40 ± 0.22 mm for left eyes (p < 0.001). Seven eyes (8.0%) had an apparent chord mu of 0.6 mm and above pre-dilatation. Fourteen eyes (16.1%) with an apparent chord mu under 0.6 mm pre-dilatation had apparent chord mu of 0.6 mm or above post-dilatation. Conclusion: Apparent chord mu length significantly increases after pharmacological pupillary dilatation. Pupil size and dilatation status should always be considered during patient selection for a planned MFIOL using apparent chord mu length as a reference marker.

Die Auswirkung von Zykloplegie auf das optische Niedrigkohärenz-Reflektometrie-Biometer ZUSAMMENFASSUNG Zweck: Das Ziel dieser Studie ist es, die biometrischen Messungen und verschiedene Formeln zur Berechnung der Intraokularlinsenstärke unter Verwendung des optischen Low-Coherence-Reflektometrie-Biometers Lenstar LS 900 vor und nach der Induktion von Zykloplegie bei Erwachsenen zu vergleichen Material und Methoden: In diese Querschnittsstudie wurden 168 Augen von 168 gesunden Freiwilligen im Alter von 40–86 Jahren (59,22 ± 11,57) eingeschlossen. Biometrische Messungen, einschließlich Achsenlänge (AL), Vorderkammertiefe (ACD), Keratometrie (K1, K2) und Weiß-zu-Weiß (WTW) wurden mit einem optischen Biometriegerät Lenstar LS 900 vor und nach Induktion einer Zykloplegie mit Cyclopentolat %1 verglichen. Die Stärke der Intraokularlinse (IOL) wurde auch unter Verwendung von sechs verschiedenen Formeln (Barrett Universal II, Haigis, SRK/T, Hoffer Q, Holladay und SRK-II) für die Intraokularlinse AcrySof MA60AC vor und nach Induktion der Zykloplegie verglichen. Ergebnisse: Es gab keine statistisch signifikanten Unterschiede in Achslänge, Keratometrie (K1 und K2) oder Weiß-zu-Weiß-Messungen vor und nach Induktion der Zykloplegie. Es gab nur einen signifikanten Anstieg der ACD aus den biometrischen Parametern nach der Induktion der Zykloplegie (P < 0,05). Trotz dieser Änderung gab es keine signifikanten Änderungen bei den Berechnungen der IOL-Stärke unter Verwendung der sechs verschiedenen Formeln vor und nach der Induktion der Zykloplegie. Schlussfolgerungen: Diese Studie zeigte, dass IOL-Stärkemessungen mit dem Lenstar LS 900 nach Zykloplegie durchgeführt werden können. Objective: The study aimed to evaluate the effect of cycloplegia on the ocular biometric parameters and intraocular lens (IOL) power calculated by Sanders–Retzlaff–Kraff/theoretical (SRK/T) formula in the cataractous eyes. Materials and Methods: This cross-sectional study included 68 senile cataractous eyes of 68 patients scheduled to undergo cataract surgery. Measurements of anterior chamber depth (ACD), axial length (AL), white to white (WtW) diameter, keratometry (K1, K2, Kmean), central corneal thickness (CCT), and pupil size (PS) were obtained with AL-Scan (Nidek Co., Ltd, Gamagori, Japan) before and after cycloplegia. The SRK/T formula was used to calculate IOL power with target refraction of 0 D. Cycloplegia induced by cyclopentolate hydrochloride %1. Results: The mean age of the patients was 70.60±4.07 years (range 65 to 80). A significant increase was observed in ACD after cycloplegia (p< 0.001). There was no statistically significant difference between pre-dilation and post-dilation AL, WtW, CCT, K1, K2, Kmean, and IOL power readings (p> 0.05). Two cases observed a decrease above 0.5 D of IOL power after cycloplegia. Conclusion: Cycloplegia induced by cyclopentolate hydrochloride %1 does not affect the measurement of the AL, WtW, CCT, keratometry, and SRK/T calculated IOL power except ACD in senile cataractous eyes.
