The Clinical and Technical Evaluation of a Remote Telementored Telesonography System During the Acute Resuscitation and Transfer of the Injured Patient

Abstract: The remote real-time guidance or observation of an EFAST using TS appears feasible. Most technical problems were quickly overcome. Further evaluation of this approach and technology is warranted in more remote settings with less experienced personnel.

“…32 Assuming instant availability of a trained physician at any time to "tele-mentor," this method could be both accurate and feasible. [33][34][35][36][37][38][39] However, limitations include mentor availability, a need for high-bandwidth Internet or cellular connections to maintain image quality, and technical problems such as image freezing. 39,40 Its effectiveness in clinical use has not yet been studied in rural Canada.…”

“…Translation to regular practice, however, will require the im mediate availability of appropriately trained remote experts willing to serve as mentors. 3 While being a content expert in a pertinent discipline is necessary to telementor remote providers to address a particular clinical problem, this expertise in and of itself is not wholly sufficient to designate someone a telementoring specialist. The ability to telementor effectively requires communication skills flexible enough to enable the remote onsite provider to perform to the best of their ability.…”

“…In Taiwan, the use of video-telemedicine significantly reduced the frequency of unnecessary air medical transports from 16 31]. Twenty total patients were analyzed with successful recognition of hemoperitoneum in five cases and pneumothoraces in two patients, not to mention the great educational benefits that accompanied the use of this teletrauma technology [31]. Finally, military medicine has been very interested in the potential of teletrauma given the obvious logistical problems involved during wartime.…”
