ÖzPurpose: Ramipril is effective in treating inflammatory myocardial injury by reducing cytokines such as TNF-α, IL-6, and IL-8. In this study, we investigated the effect of cytokine gene polymorphisms on inflammatory response which might be reduced by ramipril. Material and Methods: Of 102 patients undergoing onpump coronary artery bypass grafting surgery, 51 were the treatment group which received ramipril and the remaining 51 were the non-treated control group. TNF-α, IL-6, and IL-8 were measured with Enzyme-Linked Immuno Sorbent Assay (ELISA), before anesthesia induction (t1), at 20 min following cross clamping (t2), at the end of the operation (t3), and 24 hours after anesthesia (t4). Genotyping was performed with PCR method. Results: While TNF-α increase began with surgery, IL-6 and IL-8 increase began with cardiac arrest during cardiopulmonary bypass and continued until the end of the operation. In contrast, only IL-8 remained high in the control group during the postoperative period, while TNF-α, IL-6, and IL-8 began to decrease. The decreases in IL-6 at t3 and in TNF-α at t4 were significant. Conclusion: Ramipril might have a role in preventing inflammatory myocardial injury by reducing cytokine and TnT levels after cardiac arrest. Amaç: Ramipril, inflamatuar miyokardial hasarın tedavisinde TNF-α, IL-6 ve IL-8 gibi sitokin seviyelerini azaltarak etkili olmaktadır. Çalışmamızda, ramipril tedavisinin, koroner arter bypass cerrahisi esnasında oluşan inflamatuar yanıt ve miyokardiyal hasara yönelik etkisi üzerinde, sitokin gen polimorfizmlerinin herhangi bir rolü olup olmadığını araştırdık. Gereç ve Yöntem: Çalışmamıza, koroner arter bypass cerrahisi uygulanan hastalardan cerrahi öncesi ramipril alan 51, kontrol grubu olarak ise ramipril almayan 51 olmak üzere toplam 102 hasta dahil edildi. Her iki grupta, anestezi başlamadan hemen önce (t1), kros klemp sonrası 20. dakikada (t2), ameliyatın sonunda (t3) ve anestezinin başlangıcından 24 saat sonra (t4) alınan kan örneklerinden TNFα, IL-6, IL-8 ve TnT serum düzeyleri ELISA yöntemi ile, genotipleme ise PCR-RFLP yöntemleri ile belirlendi. Bulgular: Her iki grupta TNF-α artışı cerrahi ile başlarken, IL-6 ve IL-8 artışı kardiyopulmoner bypass sırasında kardiyak arrestle başlayıp ameliyat sonuna kadar devam etmiştir. Postoperatif dönemde ise sadece IL-8 kontrol grubunda yüksek kalırken; çalışma grubunda TNFα, IL-6, IL-8 düşüşe geçmektedir. t3 zaman aralığında IL-6, t4 zamanında TNFα değerlerinde anlamlı azalma gözlenmiştir. Sonuç: Ramiprilin kardiyak arrest sonrası sitokin ve TnT düzeylerini azaltarak inflamatuar miyokardiyal hasarı önlemede yeri olabilir.

Background Blood flow restoration is a definitive therapy for salvaging the myocardium following ischemic injury. Nevertheless, the sudden restoration of blood flow to the ischemic myocardium can induce ischemia-reperfusion injury (IRI). Results Herein, we investigated the cardioprotective effect of remote ischemic postconditioning (RPostC) through our in vivo rat model of myocardial IRI. The study included three groups: the control group, the IRI group, and the IRI + RPostC group. Ischemia-reperfusion treatment led to an increase in the myocardial infarction area, which was inhibited by RPostC. In contrast to that in the control group, the myocardial apoptosis level was enhanced in the IRI group, whereas RPostC treatment decreased IRI-induced cellular apoptosis. Affymetrix Rat Gene 2.0 ST chip data identified a total of 265 upregulated genes and 267 downregulated genes between the IRI and IRI + RPostC groups. A group of differentially expressed noncoding RNAs (ncRNAs), such as MTA_TC0600002772.mm, MTA_TC1300002394.mm, U7 small nuclear RNA (Rnu7) and RGD7543256_1, were identified. Gene Ontology (GO) enrichment analysis indicated that the positive regulation of some molecular functions, such as GTPase activity, GTP binding, cyclic-nucleotide phosphodiesterase activity and cytokine activity, may contribute to the cardioprotective role of RPostC. Moreover, pathway enrichment analysis using the Kyoto Encyclopedia of Genes and Genomes (KEGG) suggested the potential implication of the TNF signaling pathway and Toll-like receptor signaling pathway. Global signal transduction network analysis, co-expression network analysis and quantitative real-time polymerase chain reaction analysis further identified several core genes, including Pdgfra , Stat1, Lifr and Stfa3. Conclusion Remote ischemic postconditioning treatment can decrease IRI-mediated myocardial apoptosis by regulating multiple processes and pathways, such as GTPase activity, cytokine activity, and the TNF and Toll-like receptor signaling pathways. The potential role of the above ncRNAs and core genes in IRI-induced cardiac damage merits further study as well. BackgroundPredictive biomarkers of efficacy and toxicity of bevacizumab have not yet been validated. This study assessed the influence of IL-8, eNOS and VEGF-A polymorphisms in RAS mutated metastatic colorectal cancer patients receiving bevacizumab-based chemotherapy.Methods120 patients treated with first-line combination FOLFOX6 plus bevacizumab were included. A historical cohort of 112 RAS mutated colorectal cancer patients treated with FOLFOX6 alone served as control group. The following SNPs were analyzed: IL-8 c.-251T>A; eNOS c.-786T>C and c.-894G>T; VEGF-A c.936C>T, c.958T>C, c.1154A>G and c.2578C>A. Correlation of SNPs, baseline IL-8 serum levels and bevacizumab-efficacy was done.ResultsIn the bevacizumab group, carriers of the IL-8 alleles c.-251TA+AA showed a shorter PFS (P=0.002) and OS (P=0.03) compared to TT alleles. Patients with pre-treatment IL-8 < 18.25 pg/ml showed significantly longer median PFS and OS (PFS: 10.9 vs 7.6 months, P=0.005; OS: 30.7 vs 18.2 months, P<0.001) compared to patients with IL-8 higher levels (>18,25 pg/ml). IL-8 c.-251TA+AA carriers had significantly higher IL-8 levels (P<0.0001). Multivariate analysis confirmed association of IL-8 polymorphism with PFS, and of IL-8 baseline levels with both PFS and OS. IL-8 SNP did not affect the outcome in the control group. The eNOS polymorphism c.-894G>T was found associated with higher severe toxicity (P=0.0002) in patients carrying the c.-894TT genotype.ConclusionsAlthough our data need prospective validation, IL-8 and eNOS SNPs may be have a role as predictive biomarkers for bevacizumab efficacy and toxicity.
