Task Shifting and Skin Punch for the Histologic Diagnosis of Kaposi's Sarcoma in Sub-Saharan Africa: A Public Health Solution to a Public Health Problem

Abstract: Fueled by HIV, sub-Saharan Africa has the highest incidence of Kaposi's sarcoma (KS) in the world. Despite this, KS diagnosis in the region is based mostly on clinical grounds. Where biopsy is available, it has traditionally been excisional and performed by surgeons, resulting in multiple appointments, follow-up visits for suture removal, and substantial costs. We hypothesized that a simpler approach - skin punch biopsy - would make histologic diagnosis more accessible. To address this, we provided training an… Show more

“…In the field of dermatology, unlike in HIV or obstetrics (5356), there are few examples of task shifting in resource-poor settings, but the limited application of this technique has been successful. In one study, US dermatologists taught physicians, nurses, clinical officers and technicians in East Africa how to perform bunch biopsies, allowing for same day KS biopsies (57). As adapted from the mental health field (48), an apprenticeship model could allow dermatologists to train community health workers in resource limited settings on diagnosis and treatment of the most common dermatological conditions, such as dermatitis, psoriasis, acne, and tinea.…”

“…Recently, US dermatologists employed task shifting to train East African health care workers to perform punch biopsies, enabling rapid diagnosis of Kaposi sarcoma. 2 “Capacity building” and “community dermatology” also encompass elements of task shifting. Community dermatology, initially a training program for health care workers in Mexico, refers to the concept of educating and training community members to reduce the local burden of skin disease.…”
