Systematic classification of maternal and fetal intervention‐related complications following open fetal myelomeningocele repair – results from a large prospective cohort

Abstract: Objective To systematically categorise all maternal and fetal intervention‐related complications after open fetal myelomeningocele (fMMC) repair of the first 124 cases operated at the Zurich Centre for Fetal Diagnosis and Therapy. Design A prospective cohort study. Setting Single centre. Population Mothers and fetuses after fMMC repair. Methods Between 2010 and 2019, we collected and entered all maternal complications following fMMC repair into the Clavien–Dindo classification. For fetal complications, a class… Show more

“…The rate of maternal and fetal complications was similar between both study groups. Noticeable, a non‐significant trend for a higher rate of placental abruption was observed in the late intervention group and therefore, although this complication has also been described in various surgical approaches within the MOMS time window, 23,24 fetal interventions above 26 weeks should be performed with caution and in the most experienced centers with a multidisciplinary team.…”

“…A recent example used fetal AE MedDRA terms to define maternal and fetal complications after fetal surgery for myelomeningocoele. 68 The Task Force on Research Specific to Pregnant Women and Lactating Women (PRGLAC) report to the US Secretary, Health and Human Services Congress, September 2018, identified the difficulties with developing safe and effective therapies for pregnant and lactating women, due to gaps in knowledge and research from limited existing scientific literature. 69 These new AE definitions and grading in MFAET version 1.0 will provide the nomenclature to fulfil their objectives which include prioritising data collection on therapeutic products.…”
