Persons with multiple syphilis reinfections may play an important role in syphilis transmission. We analysed all syphilis tests carried out for people attending the HIV/sexually transmitted infection (STI) clinic at the Institute of Tropical Medicine, Antwerp, Belgium, from 1992 to 2012 to evaluate the extent to which syphilis reinfections were contributing to the syphilis epidemic in Antwerp. We then characterised the features of the syphilis infections in individuals with five or more episodes of syphilis. A total of 729 syphilis episodes were diagnosed in 454 persons. The majority of syphilis episodes occurred in people who had more than one episode of syphilis (445/729; 61%). A total of 10 individuals had five or more episodes of syphilis diagnosed over this period. All were men who have sex with men, HIV positive and on antiretroviral therapy. They had a total of 52 episodes of syphilis diagnosed and treated. In 38/42 of the episodes of repeat syphilis in these 10 individuals, they presented without any signs or symptoms of syphilis. Given that the majority of cases of incident syphilis in our clinic were persons with reinfections and that they frequently presented without signs of symptoms of syphilis, there is a strong case for frequent and repeated screening in all persons with a diagnosis of syphilis.

INTRODUÇÃOA sífilis é uma das principais doenças sexualmente transmissí-veis (DST), sendo causada pela espiroqueta Treponema pallidum. Manifesta-se com lesões de pele e mucosas, podendo haver acometimento sistêmico. O conhecimento da história natural da doença facilita o diagnóstico correto, os exames laboratoriais a serem solicitados, o tratamento a ser instituído e o acompanhamento posterior, os quais variarão com o estágio da doença. Em geral, passa por três estágios: primário, secundário e terciário. Entre essas fases podem ocorrer períodos de silêncio clínico, apenas com reações sorológicas positivas.A sífilis primária apresenta um cancro único, indolor, bem delimitado, que surge no local de inoculação em média 3 semanas após a infecção e tem resolução em 3 a 6 semanas, podendo estar acompanhado de adenopatia inguinal unilateral ou bilateral (1) . Pacientes com sífilis secundária podem apresentam sintomas sistêmicos que incluem cefaleia, febre, anorexia, perda de peso, dor de garganta e mialgia. Os principais sinais dermatológicos desta fase incluem exantema maculopapular, micropoliadenopatia generalizada, condiloma plano, placas mucosas e alopecia (2)(3)(4)(5) . A fase terciária, por sua vez, é caracterizada por lesões gomosas, associadas a manifestações sistêmicas como doenças cardiovasculares e neurológicas. Esta grande variedade de manifestações clínicas fez com que a sífi-lis passasse a ser reconhecida como doença "imitadora".É doença transmitida por via sexual (sífilis adquirida) e verticalmente (sífilis congênita). O contato com as lesões contagiantes (cancro duro e lesões secundárias) pelos órgãos genitais é responsável por 95% dos casos de sífilis. O risco de contágio varia de 10 a 60%, conforme a maioria dos autores.O encurtamento dos períodos de silêncio clínico é comumente visto em pacientes que apresentam coinfecção com o vírus da imunodeficiência humana (HIV), havendo a possibilidade de o cancro persistir durante o estágio secundário (6)(7)(8)(9) . Não é usual tal alteração cronológica em pacientes sem essa coinfecção.Apresentamos dois casos de pacientes com persistência de cancro duro no secundarismo sifilítico e alertamos para a perda de qualidade no atendimento médico quando uma anamnese cuidadosa e uma boa avaliação clínica não são aplicadas, principalmente em DST. RelaTO De CasO Caso 1Paciente masculino, 23 anos de idade, natural e residente da Etiópia, apresentou, há 4 semanas, história de uma úlcera indolor na glande peniana e de 1 semana de exantema maculopapular. O paciente relatou ter múltiplos parceiros sexuais e não usar preservativo. Ao exame dermatológico, havia uma úlcera única, indolor, com fundo limpo, localizada na glande (Figura 1), além de inú-meras pápulas localizadas na fenda glútea, bolsa escrotal, região ResUMOA sífilis é uma doença infectocontagiosa de alta prevalência, especialmente nos países em desenvolvimento, e costuma ser denominada a "grande imitadora", devido seu polimorfismo lesional. SIR, Syphilis is a sexually-transmitted infectious disease caused by the bacteria Treponemapallidum. A characteristic manifestation of the third stage is arteritis of the aorta, which leads to necrosis and loss of tissue. Here, we report on a 48-year-old woman admitted with suspected Takayasu's arteritis (TA) due to localized inflammation confined to the aortic root and valve, which could be diagnosed as syphilitic aortitis. KeywordsVasculitis, Syphilis, Takayasu's Arteritis Letter to the EditorA 48-year-old patient was admitted to our tertiary Rheumatology center after composite aortic valve graft replacement had been performed the month before due to 70% stenosis of the aorta combined with an aortic valve insufficiency. The patient had developed 4 months prior to admission rapid signs of cardial decompensation. The pathohistological analysis of the resected tissue revealed aortitis with lympho-plasma cell rich infiltrations around vasa vasorum and lymphatic aggregates in the adventitia (Figure 1). In addition, there was a thickened vessel wall with slight fibredesorganisation and intramural lamelliform necrosis of the media with sparse neutrophilic granulocytes, lymphocytes and plasma cells (Figure 2). Immunohistochemical studies including stainings for CD68, IgG4 and CD138 did not provide additional information. Off note, there were no granulomas present. In light of these histopathological findings, infectious aortitis as well as localized idiopathic aortitis were considered by the pathologists, however, no information regarding an infection has been provided on request
