Syphilis serology today

Abstract: Serologic testing is an important adjunct in the diagnosis and management of syphilis. In this review, the various currently used serologic tests, both non-treponemal and treponemal, are considered. Special emphasis is placed on their relative advantages, applications, and limitations. Conditions under which these tests may be falsely positive are discussed. Additional diagnostic tools cited are the dark-field examination, for use in early-lesion syphilis, and cytological and immunochemical techniques for dete… Show more

“…Sensitivity of the T. pallidum immobilization test is 41-56% in primary, 94-97% in secondary and 92-98% in late syphilis. The haemagglutination tests have sensitivity similar to that of the FTA-ABS, except in primary syphilis in which non reactivity occurs in up to 36% of cases [16]. False-positive results have been reported in auto-immune diseases, alcoholic cirrhosis, lymphosarcoma, vaccination and individuals with no evidence of any disease.…”

“…Para a sífilis a prevalência calculada de 4,1% poderia ser considerada superestimada devido a inespecificidade do VDRL por ser uma reação não treponêmica com possibilidade de resultados falso positivos 11,24 . Por este motivo e pela instabilidade do resultado do teste a validação da prevalência torna-se bastante difícil.…”

“…The apparent lack of utility of VDRL testing may be explained by the truly rare occurrence of cerebral neurosyphilis. However, the use of the VDRL test in screening for cases of late syphilis has been questioned, since up to one‐quarter of afflicted patients have a falsely negative result 11,12 . The FTA‐ABS test, with a reported sensitivity of 95 per cent, has been recommended as the preferable screening test in this setting 13 .…”
