Abstract: Surgery remains the best curative option in patients with early stage lung cancer (stage I and II). Developments in minimally invasive techniques now allow surgeons to perform lung resections on elderly patients, patients with poor pulmonary function or significant cardiopulmonary comorbidities. New techniques, such as stereotactic radiotherapy and ablative procedures, are being evaluated in early-stage lung cancer and may represent an alternative to surgery in patients unfit for lung resection. Perioperative… Show more

“…[8][9][10]19 VATS lobectomy with systematic lymph node dissection has been widely accepted as a safe and effective choice for early-stage lung cancers, and has now been recommend by the American College of Chest Physicians' evidence-based clinical practice guidelines. 20 Elderly patients with lung cancer face more comorbidity or poor cardio-pulmonary function and are, therefore, correlated with a higher possibility of postoperative complications.…”

“…8 Lobectomy by thoracotomy (TL) has been the "golden standard" for early stage non-small cell lung cancer (NSCLC) compared with other treatment options. 9,10 However, thoracotomy is not a suitable treatment option for elderly patients, as many carry chronic diseases, such as diabetes, hypertension, and coronary heart disease, and, subsequently, have poor cardio-pulmonary function. 11,12 Video-assisted thoracoscopic (VATS) lobectomy, which has developed significantly during the past decade and become widely accepted, has provided opportunities to this category of patients because of the advantage of minimal invasion.…”

“…The other option is to perform tentative extended resections. However, this might lead to loss of more lung function and worsen the patient's long-term prognosis (13,14). Thus, it is of great importance to find a practical method to avoid conversion to extended resection.…”

“…Approximately 85% of lung cancers are nonsmall cell lung cancers (NSCLC) [2,3] , and although the number of surgical candidates has increased [1] , only 15-30% of the patients are suitable for surgical resection with a curative intent [2,4] .…”

“…Surgical resection is considered the standard treatment for early-stage disease (stage I and II) [2,[7][8][9][10] and is also an accepted treatment modality in some locally advanced disease (stage IIIA) [2] .…”
