Surgery for Colorectal Liver Metastases

Abstract: Half of all patients with colorectal cancer develop metastatic disease. The liver is the principal site for metastases, and surgical resection is the only modality that offers the potential for long-term cure. Appropriate patient selection for surgery and improvements in perioperative care have resulted in low morbidity and mortality rates, resulting in this being the therapy of choice for suitable patients. Modern management of colorectal liver metastases is multimodal incorporating open and laparoscopic surg… Show more

“…1 Despite this, surgical resection of metastatic sites of disease remains the only potentially curative therapy. 25 Five-year survival rates of 40% and as high as 54% in selected patients have been reported with pulmonary and liver metastasectomy. 68 Although improved survival has been demonstrated with metastatic site resection, uptake of aggressive surgical approaches remains low among certain patients.…”

“…However, they defined metastatic involvement on CT differently depending on position of the nodal station. Portocaval nodes were considered involved if cross product of dimensions was ≥0.65 cm 2 . Pancreaticoduodenal and hepatic artery nodes were considered involved as long as CT could detect them.…”

“…About 50% of patients with colorectal carcinoma will eventually develop metastases, with liver and lungs being the most common sites [2] [3] [4] [5] [6]. For patients with liver metastases, surgery remains the gold standard of treatment with 5-year survival rates approaching 50% [6]- [11].…”

“…Nowadays, patients with liver metastases are treated with neoadjuvant chemotherapy and then undergo local resection [4]. This treatment has a 5-year survival rate of 30–58%, with a high rate of recurrence [3, 4, 5, 6]. If the metastases are unresectable, the survival rate does not exceed 2% [4].…”
