Superior Mesenteric Artery (Wilkie's) Syndrome as a Result of Cardiac Cachexia

Abstract: Superior mesenteric artery (SMA) syndrome is a rare acquired disorder in which acute angulation of SMA causes compression of the third part of the duodenum between the SMA and the aorta, leading to obstruction. Loss of fatty tissue as a result of a variety of debilitating conditions is believed to be the etiologic factor causing the acute angulation. We report a case of an 86-year-old man with prolonged congestive heart failure and aortic stenosis in which SMA syndrome developed as a result of cardiac cachexia… Show more

“…Weight loss results in loss of the mesenteric fat pad and the superior mesenteric artery compresses duodenum. Bodyweight loss with superior mesenteric artery syndrome, including eating disorders, cardiac cachexia, HIV patients, hereditary motor and sensory neuropathy, have been reported [7][8][9][10] . The radiographic studies used to establish diagnosis include an upper gastrointestinal series, computed tomography (CT), CT angiography, conventional angiography, abdominal sonography, and magnetic resonance angiography (MRA) [2,[11][12][13] .…”

“…Superior mesenteric artery (SMA) syndrome which is also known as Wilkie's syndrome, is a rare cause of upper gastrointestinal obstruction [1][2][3]. Superior mesenteric artery syndrome was first described in 1861 by von Rokitansky, who proposed that its cause was obstruction of the third part of the duodenum as a result of arteriomesenteric compression [4].…”
