Sulfasalazine and pentoxifylline in psoriasis: A possible safe alternative

Abstract: Although incomparable to methotrexate, combined sulfasalazine and pentoxifylline produced a good response in cases of extensive psoriasis. Multicentre studies are needed to validate these results.

“…The kidney is an organ that is highly vulnerable to damage caused by ROS, probably due to the abundance of polyunsaturated fatty acids in the composition of renal lipids (6). Pentoxifylline is a low-cost drug with antitumor necrosis factor α (TNF-α) properties (7). This drug is used in the treatment of many diseases such as cardiovascular disorders, diabetic nephropathy, alcoholic hepatitis, and venous leg ulcer (8).…”

“…In a study of 52 patients with generalized lichen planus, the most pruritus relieving effect was seen in the first 3 weeks of treatment 9 . Sulfasalazine resulted in a mean 57.09% reduction in PASI score at 8th week in a small psoriasis trial and was associated with promotion and maintenance of hair growth in 6 patients with alopecia areata after 2‐12 months of use 10,11 . Sulfasalazine, to our knowledge, has never been reported as a therapeutic option in EPD.…”

“…Pentoxifylline is not only compatible with other drugs used for the management of AS, but it is also postulated to act synergistically to reduce TNF-α levels through independent mechanisms with sulfasalazine. Their effectiveness has been shown in other autoimmune disorders such as pemphigus vulgaris, entero-Behcet's disease, and psoriasis [26][27][28]. This is particularly important in the geographical area where the authors of the current study practice because of the higher prevalence of infectious diseases such as TB and the relatively lower socioeconomic status of the population, both of which make the prescription of anti-TNF agents an even more challenging issue and the search for alternatives a necessary area of research.…”
