Sudden Cardiac Death Unresponsive to Implantable Defibrillator Therapy: An Urgent Target for Clinicians, Industry and Government

Abstract: A major expansion in utilization of implantable cardioverter-defibrillators (ICDs) is anticipated based on the results of randomized clinical trials (RCT) that demonstrate increased survival in a sizable population of patients with reduced left ventricular function. However, if RCT accurately reflect clinical practice, then a substantial proportion of patients will die suddenly despite ICD implantation. ICD-unresponsive sudden cardiac death (SCD) has been recognized since the initial ICD experience. Yet, despi… Show more

“…[1][2][3][4]6 Catheter ablation has been shown to reduce arrhythmia recurrence in patients with VT; however, no study has been powered to test whether prevention or a significant reduction of ICD therapies by CA improves survival. [14][15][16][17] In the early Cooled RF trial, which included only patients with hemodynamically stable VT, CA was successful, as defined by elimination of all mappable VTs, in 41%.…”

“…ICD dysfunction may result in failure to deliver therapy for ventricular tachycardia and thus result in syncope or sudden death. Pooled data of randomized clinical trials that involve around 3500 patients indicate an ICD-unresponsive sudden-death rate of nearly 5% (Anderson, 2005). Primary ventricular arrhythmia detection is based on the tachycardia detection rate which is usually programmed with a safety margin of 20 beats/min below the clinical arrhythmia.…”

“…This may be more effective than any other available therapy and is thought to be additive to reduction of SCD due to other therapies such as beta-blockers and angiotensin converting enzyme inhibitors. On the other hand, it should serve as a reminder to physicians of the importance of optimization of ICD programming and medical therapy and to patients with regard to compliance with medications and recommended life-style modifications as these measures will reduce their risk (Anderson, 2005). The second explanation is a possible non-direct damage on the myocardium for the adverse effects of ICD shocks that could direct to increased mortality.…”
