Successful recovery of tachycardia‐induced cardiomyopathy with severely depressed left ventricular systolic function by catheter ablation with mechanical hemodynamic support: a case report

Abstract: Key Clinical MessageWe describe the case that persistent atrial fibrillation refractory to rhythm control by pharmacotherapy and electrical cardioversions caused tachycardia‐induced cardiomyopathy with low ejection fraction and hemodynamic instability. Mechanical hemodynamic support using an intra‐aortic balloon pump is one of the choices of hemodynamic support during catheter ablation by pulmonary vein isolation.

“…Kamada et al in 2016 described a case of tachycardia‐induced cardiomyopathy secondary to refractory AF/RVR to rhythm control. PVI/PWI was successfully achieved, but the MCS used was an intraaortic balloon pump 10 . In contrast, our patient had Impella CP at the start of the procedure.…”

“…In most cases, supraventricular arrhythmias are hemodynamically stable and well-controllable with medical treatment. However, in few cases, mechanical circulatory support was needed to control such arrhythmias, which are refractory to medical therapy [3,4]. We describe here a case of life-threatening SVT storm associated with theophylline toxicity, in which mechanical support, as well as continuous hemodiafiltration (CHDF), was effective.…”
