Subthalamic Nucleus Stimulation in Severe Obsessive–Compulsive Disorder

Abstract: These preliminary findings suggest that stimulation of the subthalamic nucleus may reduce the symptoms of severe forms of OCD but is associated with a substantial risk of serious adverse events. (ClinicalTrials.gov number, NCT00169377.)

“…This effect was the opposite to what would have been expected from animal experiments showing that STN inactivation led to impulse control disorder, which could be considered OCD-like behaviour [106]. The clinical observations in three PD patients prompted a crossover double-blind study of DBS in OCD that targeted the limbic-associative part of the STN [21]. In this trial, 16 patients were randomized and participated in two 3-month treatment phases, either active STN DBS or sham stimulation, separated by a washout period.…”

“…The OCD severity was significantly lower after active STN DBS than after sham stimulation. Interestingly, STN DBS provoked reversible disinhibited behaviour that could be controlled by a decrease in stimulation intensity [21].…”
