Abstract: Objective To evaluate the association of subretinal hyper-reflective material (SHRM) with visual acuity (VA), geographic atrophy (GA) and scar in the Comparison of Age related Macular Degeneration Treatments Trials (CATT) Design Prospective cohort study within a randomized clinical trial. Participants The 1185 participants in CATT. Methods Participants were randomly assigned to ranibizumab or bevacizumab treatment monthly or as-needed. Masked readers graded scar and GA on fundus photography and fluoresce… Show more

“…SHRM, a morphologic feature seen on optical coherence tomography (OCT) as hyperreflective material located external to the retina and internal to the retinal pigment epithelium (RPE), is associated with reduced VA and increased scar formation. 28,29 The CATT cohort had either CNV or its sequelae in the foveal center and eyes with RAP tended to have CNV that was predominantly extra-foveal. As a result deleterious morphological outcomes such as GA and scar tended to occur in an extra-foveal location.…”

“…29 SHRM is a negative prognostic biomarker in AMD and has been used as an endpoint for clinical trials. 30 The potential source of material contributing to SHRM includes fibrovascular scars, blood, exudate, cholesterol, and other AMD components including undigested outer segment debris. 31-34 Differences in appearance of this material on BL-AF and NI-AF may represent variations in pigmentary changes and lipofuscin content.…”

“…12 Although its exact composition is unclear, SHRM was reported to influence scar development and sustained loss of visual acuity in the Comparison of Agerelated macular degeneration Treatments Trials (CATT) study. 13,14 Anti-VEGF therapies result in marked morphologic improvements that can occur rapidly, but there is little information on the composition of SHRM and its evolution and rate of disappearance.…”
