“…The effectiveness of a subcutaneously implanted E 2 pellet in preparing the endometrium for an OD cycle was tested in 13 women who had previously been submitted to what the investigators call a standard E replacement (ER) regimen (44). These previous cycles were diagnosed as showing inadequate endometrial growth.…”

“…It is interesting that the endometrial thickness observed sonographically (6.1 mm) was less than what is ordinarily observed for patients with estradiol values approaching 1000 pg/mL. Notably, the pregnancy rates using donor oocytes in patients with POF are below expected (14), and others have observed endometrial responses to exogenous estrogens that are similarly suboptimal in this patient population (15). The mechanisms of hormone resistance in this patient population are not well defined but may include chronic changes in uterine vascularization, deficient production of unknown ovarian factors other than steroid hormones, and relative defects in the estrogen signaling cascade.…”

“…In contrast, an increase in endometrial thickness by supplementing with vaginal E 2 (4 mg/day) was refuted by a prospective trial (161). Subcutaneous E 2 pellets may be used for thin endometrium as was reported by a small scaled oocyte donation study (162); iii) modification of the type of E 2 . A RCT compared vaginal synthetic estrogen (n=30) with vaginal natural estrogen (n=30) in patients with refractory thin endometrium.…”
