Summary:Although eosinophilia after stem cell transplantation (SCT) has been addressed in recent reports, the significance of eosinophilia in disease outcome after SCT has not been well studied. In this study, we investigate the frequency of eosinophilia after SCT to determine its prognostic value. The subjects were 113 patients with malignant or nonmalignant diseases who underwent SCT treatment. In these patients, eosinophilia was detected in 44 cases (38.9%), on average 67.5 days after transplantation, and the mean maximum absolute eosinophil count was 840.5 Â 10 6 /l. To study the basis of eosinophilia after SCT, various serum cytokine levels during SCT in patients both with and without eosinophilia were analyzed. Statistical analysis indicated that the overall patient survival rates improved in those with eosinophilia compared to those without eosinophilia (88.7 vs 43.0%, P ¼ 0.0034). In particular, in patients with malignant diseases, those with eosinophilia showed a higher eventfree survival (81.1 vs 44.6%, P ¼ 0.0025) and a lower relapse rate (16.0 vs 43.0%, P ¼ 0.0287) than those without eosinophilia. In conclusion, we propose that eosinophilia after SCT could be a useful prognostic marker for determining favorable outcomes in patients with malignant diseases. The reasons for this good prognosis in SCT patients with eosinophilia are discussed. Bone Marrow Transplantation (2005) 36, 985-991.

Doxorubicin is a potent chemotherapeutic drug; however, its clinical application has been limited due to its cardiotoxicity. One of the major mechanisms of doxorubicin-induced cardiotoxicity is the induction of oxidative stress. Evidence from in vitro and in vivo studies demonstrates that melatonin attenuated the increase in ROS production and lipid peroxidation from doxorubicin. Melatonin has been shown to exert protective effects on mitochondria damaged by doxorubicin via attenuating the depolarization of the mitochondrial membrane, restoring ATP production, and maintaining mitochondrial biogenesis. Doxorubicin increased mitochondrial fragmentation which impaired mitochondrial function; however, these adverse effects were reversed by melatonin. Melatonin also modulated cell death pathways by suppressing apoptotic and ferroptotic cell death caused by doxorubicin. These beneficial effects of melatonin could be responsible for the attenuation of changes in ECG, left ventricular dysfunction, and hemodynamic deterioration caused by doxorubicin. Despite these potential benefits, clinical evidence regarding the impact of melatonin in reducing cardiotoxicity induced by doxorubicin is still limited. Further clinical studies are justified to evaluate the efficacy of melatonin in protecting against doxorubicin-induced cardiotoxicity. This valuable information can be used to warrant the use of melatonin in a clinical setting under this condition.

Cancer is one of the most prevailing causes of mortality worldwide. Standard therapies for cancer patients include surgical intervention, chemotherapy, and radiotherapy. Nevertheless, chemoresistance and toxicity are clinical implications of advanced disease. New treatments are obliged to overcome these challenges. According to clinical investigations, melatonin (MLT) has the potential to prevent and cure cancer. It is nontoxic and has a plethora of anticancer properties including apoptosis, antiangiogenic, antiproliferative, and metastasis‐inhibitory mechanisms. It enhances the therapeutic sensitivity of malignancies when coupled with conventional medications. The investigations that demonstrate how MLT possesses antitumor characteristics are reviewed in this manuscript. We provided an overview of recent research on the etiology, factors associated, therapeutic property of MLT on various cancer types. We discussed the clinical implications of MLT alone or in conjunction with chemotherapeutic drugs or radiation therapy. Furthermore, we have addressed the mechanisms of its anticancer activities against several types of cancers along with research findings of various investigations done by researchers and advancements in the field of nanotechnology for efficient delivery of MLT. The recent clinical investigations of MLT in various cancer types are reported in this review.
