Patients presenting to the emergency department (ED) or clinic with acute exacerbation of asthma (AEA) can be very challenging varying in both severity and response to therapy. High-dose, frequent or continuous nebulized short-acting beta2 agonist (SABA) therapy that can be combined with a short-acting muscarinic antagonist (SAMA) is the backbone of treatment. When patients do not rapidly clinically respond to SABA/SAMA inhalation, the early use of oral or parenteral corticosteroids should be considered and has been shown to impact the immediate need for ICU admission or even the need for hospital admission. Adjunctive therapies such as the use of intravenous magnesium and helium/oxygen combination gas for inhalation and for driving a nebulizer to deliver a SABA and or SAMA should be considered and are best used early in the treatment plan if they are likely to impact the patients' clinical course. The use of other agents such as theophylline, leukotriene modifiers, inhaled corticosteroids, long-acting beta2 agonist, and long-acting muscarinic antagonist currently does not play a major role in the immediate treatment of AEA in the clinic or the ED but is an important therapeutic option for physicians to be aware of and to consider initiating at the time of discharge from clinic, hospital, or ED to reduce later clinical worsening and readmission to the ED and hospital. A comprehensive summary is provided of the currently available respiratory pharmaceuticals approved for asthma and other airway syndromes. Clinicians must be prepared to use the entire spectrum of medications available for the treatment of acute asthma exacerbations and the agents that should be initiated to prevent worsening or additional exacerbations. They need to be familiar with the major potential drug toxicities associated with their use.

Bronchial thermoplasty is a new treatment option for patients with severe bronchial asthma who remain symptomatic despite maximal medical therapy. The aim of this interventional therapy option is the reduction of smooth muscle in the central and peripheral airways in order to reduce symptomatic bronchoconstriction via the application of heat. A full treatment with bronchial thermoplasty is divided into three bronchoscopies. Randomized, controlled clinical trials have shown an increase in quality of life, a reduction in severe exacerbations, and decreases in emergency department visits as well as days lost from school or work. The trials did not show a reduction in hyperresponsiveness or improvement in forced expiratory volume in 1 s. Short-term adverse effects include an increase in exacerbation rate, an increase in respiratory infections and an increase in hospitalizations. In the 5-year follow up of the studies available there was evidence of clinical and functional stability of the treated patients. Further studies are necessary to identify an asthma phenotype that responds well to this treatment.

Background: There are some non-invasive and affordable treatments for asthma with fewer side effects, such as reflexology and olive oil foot massage in complementary and alternative medicine. There is a lack of studies regarding the efficacy of olive oil foot massage and reflexology for asthma control. This study was done to investigate the effect of foot reflexology and olive oil foot massage on asthma control. Methods:This randomized controlled clinical trial study was performed in 2013 on 45 adult patients suffered from asthma. Participants were divided into three groups (15 in each group): foot reflexology (R), olive oil foot massage (O) and control(C). The reflexology and olive oil foot massage were done by a massage therapist for ten sessions, three times a week for 15 minutes. Asthma control questionnaire (ACQ) was used to determine the adequacy of asthma control. Data were analyzed by SPSS 18. P-value<0.05 was considered statistically significant. Results:Significant difference was seen in Asthma Control mean scores for both of reflexology and foot massage groups at the baseline and after the intervention (p<0.01). There was no significant difference between R and O groups in their asthma control mean scores at the end of the intervention. Conclusion:The results showed that foot reflexology and olive oil foot massage could improve the asthma control. Further study is needed on the interaction of these two interventions, to compare them with the use of specific drug treatments, and to investigate the effect of olive oil absorption on asthma.
