Abstract: Open surgical repair of complex aortic pathologies using cardiopulmonary bypass and deep hypothermic circulatory arrest still carries a substantial rate of mortality and morbidity. Endovascular stent-graft placement has developed as a safe and effective treatment modality for various diseases of the aorta. We report on the case of a 65-year-old female presenting with symptomatic type B aortic dissection with aneurysm of the ascending aorta and the aortic root. The patient was treated with a flanged composite g… Show more

“…There are few case reports of surgical treatment of type B dissection with ATAD, as reconstruction of the ascending thoracic aorta and repair of acute type B dissection via a median sternotomy or lateral thoracotomy using a conventional single‐stage procedure remains surgically challenging. Ascending thoracic aortic reconstruction combined with implantation of a conventional elephant trunk presents a viable treatment option for this lesion.…”

“…A hybrid approach involving repair of the ascending thoracic aorta and staged or simultaneous endografting of the descending aorta and/or the aortic arch is also a viable treatment option for this type of lesion . In spite of early satisfactory results, late complications associated with TEVAR (endoleakage, graft migration, stent fractures, and aneurysm‐related death) are much more common than when employing an open aortic surgical approach .…”

“…Open repair often involve cardiopulmonary bypass with hypothermic circulatory arrest and various cerebral preservation techniques 1-6 . Introduction and evolution thoracic endovascular aortic repair (TEVAR) techniques have contributed in treatment of a wider range of patients with higher comorbidity index 5,7 . Combining conventional surgical techniques with endovascular technology, the "hybrid" aortic arch repair minimizes the operation by either eliminating or significantly simplifying and shortening the arch reconstruction period, thus limiting the duration of circulatory arrest and cerebral ischemia.…”
