Abstract: An aorto-coronary artery bypass operation combined with left ventricular aneurysmectomy was performed successfully in a 56-year-old man who had suffered spontaneous coronary artery dissection with left ventricular aneurysm. It is important to perform myocardial revascularization surgery even if the region for anastomosis is dissected.

“…But, in cases with post infarct complications such as left ventricular aneurysm, mitral regurgitation, and the presence of other atherosclerotic coronary artery disease, the treatment should be surgical. These situations are very rare and only two cases of aneurysmectomy 11,12 and one of mitral valve plasty 13 have been reported. In all previous cases, coronary artery bypass grafting to the dissected artery was performed.…”

“…38,42,50 Coronary artery bypass grafting has also been used successfully in the treatment of spontaneous coronary artery dissection. 37,[39][40][41][43][44][45][46][47][48][49]51,52 The purpose of the surgical treatment is to restore blood flow to the true lumen and obliterate the false lumen. Both saphenous vein grafts and internal mammary artery grafts have been used with excellent results.…”
