Background: Periungual myxoid cyst is a benign cyst of the digits, usually occurs in hands although reported in toes as well. They are commonly located between distal nail crease and proximal nail fold. Aim: The aim of our study was to study the outcome of excision of periungual cyst using flap surgery. Methods: Clinical features of all six patients were recorded. All were treated with flap surgery. Intra operative findings and treatment outcome were recorded and analyzed. Results: We enrolled four male and two female patients with average age being 27+/-7.5 years. Presenting symptom was swelling over dorsal surface of distal interphalangeal joint. Nail plate deformity was seen in all patients. Follow up was done for 8 months to 1 year. Follow up period was un-eventful. Conclusion: Periungual myxoid cyst have characteristic clinical presentation. Excision using flap surgery is an excellent mode of treatment allowing better visualization, easy exploration and excellent outcome.

STRESZCZENIEWprowadzenie. Torbiel śluzowa jest łagodnym, przeziernym guzkiem zlokalizowanym na grzbietowej powierzchni palca. Wnętrze torbieli wypełnia galaretowaty materiał i pozostaje ona w łączności ze stawem międzypaliczkowym. W etiologii schorzenia znaczącą rolę przypisuje się zmianom zapalnym w stawach. Cel pracy. Przedstawienie przypadku torbieli śluzowej palca ręki oraz trudności terapeutycznych. Opis przypadku. Pacjentka 60-letnia zgłosiła się do Poradni Dermatologicznej z powodu miękkiego guzka zlokalizowanego w okolicy okołopaznokciowej palca środkowego ręki lewej. Obraz kliniczny sugerował obecność torbieli śluzowej. W leczeniu stosowano wiele metod terapeutycznych (punkcja, elektrokoagulacja, krioterapia, leczenie chirurgiczne) z różną skutecznością. Wnioski. Wybór sposobu leczenia torbieli śluzowej palca jest indywidualny, jednak najskuteczniejsze okazało się chirurgiczne usunięcie zmiany.
ABSTRACT
Introduction.Digital mucous cyst is a benign, translucent nodule that occurs on the dorsum of the finger. The cyst is filled with a gelatinous mass and connected with the interphalangeal joint. Arthritis plays the main role in the etiology. Objective. To present a case of digital mucous cyst and its therapeutic difficulties. Case report. A 60-year-old patient was admitted to the Outpatient Clinic of Dermatology because of a soft, periungual nodule on the left third finger. The clinical picture suggested a mucous cyst. Many treatment methods were used with varying therapeutic efficacy (puncture, electrocoagulation, cryotherapy, surgery). Conclusions. The choice of treatment is individual, but in our case, surgical removal of the lesion was most efficacious.
