Simultaneous Resections of Colorectal Cancer and Synchronous Liver Metastases: A Multi-institutional Analysis

Abstract: Simultaneous colorectal and minor hepatic resections are safe and should be performed for most patients with SCRLM. Due to increased risk of severe morbidity, caution should be exercised before performing simultaneous colorectal and major hepatic resections.

“…In 2004, Dindo et al proposed a modification that included a new five-grade classification system with the possibility of combining grades; ''disability'' was added to the system, ''hospital stay'' was eliminated as a complication and ''organ failure'' was changed to carry more weight in grading [12]. The so-called ''Clavien-Dindo'' classification has been adapted for use in general surgery [10] and for other surgical subspecialties such as gastroenterology/hepatology [3,11,17,[22][23][24][25]28], urology [19], and nephrology [18,27]. Given this system appears useful for general surgery and other specialties, we explored its use in orthopaedic surgery.…”

“…[5][6][7][8][9] Historically, patients were managed with resection of the primary and subsequent systemic therapy prior to consideration of hepatectomy in order to best select patients for this complex and potentially morbid procedure. However, recent evidence suggests that simultaneous colorectal/liver resection (SCLR) of the primary tumor and the liver metastases might be an attractive option, 10 but the safety of this strategy has not been explored outside of modest single or multiinstitutional experiences.…”

“…Practicar la I N V E S T I G A C I Ó N cirugía colónica y hepática diferidamente o resecar las metástasis en forma simultánea a la cirugía colorrectal. La cirugía en 2 tiempos es la opción que se ha preferido en muchos centros, aunque recientemente han aparecido grupos que proponen realizar la cirugía en forma simultánea [26][27][28][29] . Esta serie presenta 32% de resecciones simultá-neas.…”
