Severe Clinical Outcomes of Tuberculosis in Kharkiv Region, Ukraine, Are Associated with Beijing Strains of Mycobacterium tuberculosis

Abstract: Genotypic variation in Beijing lineages of Mycobacterium tuberculosis (MTB), the causative agent of tuberculosis (TB), has been associated with hyper virulence and the spread of extensively and multiple drug (X/MDR) resistant MTB strains in Eastern Europe, Central Asia, and East Asia. The clinical outcomes of 215 new cases of TB among the population of the Kharkiv region of Eastern Ukraine were analyzed to uncover factors associated with severe infection. Infecting MTB strains were profiled by 5 locus exact ta… Show more

“…Further, we show that MDR/XDR TB patients in southern Ukraine are infected mainly with 3 distinct MTBC Beijing outbreak clades. However, inhabitants of southeastern Ukraine were indeed at higher risk for primary MDR TB (36,37). This finding might be linked to the circulation of the Beijing outbreak strains we describe but we cannot extrapolate our results to the whole country which was shown to have an asymmetric distribution of MDR TB (37).…”

“…On the other hand, the L2 Beijing genotype was more frequently found among foreign-born patients (P<0.05). Overall, the high prevalence of the Beijing isolates in Poland evidenced in this study, coupled with a high proportion of the Beijing genotype among foreign-born TB patients may reflect an ongoing circulation of this genotype imported to Poland mainly from FSU countries ( Niemann et al., 2010 ; Konstantynovska et al., 2019 ). Of note, one patient born in Poland was infected with type MLVA 1071-32, which has been recently described as an emerging resistant Beijing variant that is circulating in Siberia.…”

“…Ukraine has a recent history of outbreaks of infections such as polio and measles, 19 , 20 , drug-resistant tuberculosis 21 , 22 , and numerous veterinary pathogens 23 , which are sustainably controlled in other countries. Furthermore, despite a high HIV and hepatitis C virus prevalence, infectious disease genomic epidemiology is currently not integrated into public health decision making in Ukraine 24 , 25 .…”
