Summary YKL-40, a growth factor for connective tissue cells, is secreted by cancer cells and macrophages. Elevated serum YKL-40 in patients with metastatic carcinoma has been associated with poor prognosis. We evaluated serum YKL-40 in 131 patients with small cell lung cancer (SCLC). Twenty-two percent of the patients with limited disease and 40% of the patients with extensive disease had elevated serum YKL-40. The median survival was 5.1 months for patients with elevated serum YKL-40 and 9.0 months for patients with normal serum YKL-40. Patients with elevated serum YKL-40 had increased hazard for death within the first 6 months after the start of chemotherapy compared to patients with normal serum YKL-40 (HR = 2.06, P = 0.009). Multivariate Cox analysis including routine prognostic variables showed that serum YKL-40 (P = 0.02) is independent of prognostic variables for survival within the first 6 months. Studies are needed to determine the function of YKL-40 in SCLC.

Rheumatoid arthritis is characterized by synovial joint infiltration of activated CD4+ T cells and MHC class II+ APC, and is linked to specific HLA-DR alleles. Candidate autoantigens in synovial fluid and cartilage include type II collagen (CII) and cartilage gp39 (HCgp39). Using preparations of native Ag and T cells derived from Ag-immunized DR4-transgenic mice, we determined that human ex vivo differentiated DR4+ dendritic cells (DC) and macrophages (Mφ) can mediate MHC class II presentation of CII or HCgp39 epitopes. The form of the Ag (soluble, partially degraded, or particulate) delivered to the APC influenced its presentation by DC and Mφ. DC efficiently presented partially degraded, but not native CII α-chains, while Mφ presentation was most efficient after phagocytosis of bead-conjugated CII. Both DC and Mφ presented soluble HCgp39, and activated Mφ from some donors presented epitopes derived from endogenously synthesized HCgp39. When synovial fluid from rheumatoid arthritis patients was used as a source of Ag, DC presentation of HCgp39 and CII epitopes was efficient, indicating that synovial fluid contains soluble forms of CII and HCgp39 amenable to internalization, processing, and presentation. These data support the hypothesis that CII and HCgp39 are autoantigens and that their class II-mediated presentation by DC and Mφ to T cells in vivo has a critical role in the pathogenesis of human rheumatoid arthritis.

Objective: YKL-40, a new biomarker of localized inflammation, is secreted by macrophages within the atherosclerotic plaques. Coronary artery ectasia (CAE) is a clinical entity with unclear etiopathogenesis. Some studies have revealed that CAE may be a form of atherosclerosis that has more localized and intense inflammatory properties than atherosclerosis. The goal of this study was to investigate YKL-40 and C-reactive protein (CRP) levels in patients with isolated CAE compared to patients with normal coronary arteries (NCA) and coronary artery disease (CAD). Methods: Our study has an observational and cross-sectional design. Forty-nine patients with isolated CAE (mean age: 60±10 years), 30 age-and gender-matched control participants with NCA (30 patients, mean age: 58±12 years) and 30 patients with CAD (mean age: 61±10 years), were included in the study. The relationship between YKL-40, CRP levels and the presence of CAE was investigated. Univariate and multiple logistic regression analysis were used for analysis of independent variables to predict CAE. Results: Serum YKL-40 levels were significantly different among study groups (NCA: 110±53 μg/L, CAE: 144±68 and CAD: 180±117, p=0.005). CAD group and CAE group had significantly higher YKL-40 levels than NCA group (p=0.004 and p=0.015, respectively). CRP was not significantly different between three groups. In addition, there were no any statistically significant differences, with respect to age, gender, the presence of hypertension or diabetes mellitus, and the smoking status (p>0.05). Logistic regression analysis revealed only YKL-40 level as the determinant of CAE (OR: 1.010, 95% CI: 1.001-1.019, p=0.027). Conclusion: YKL-40 levels in patients with isolated CAE compared to patients with NCA were found significantly high and only YKL-40 level was established as the determinant of CAE. We believe that further studies are needed to clarify the possible causative roles of YKL-40 in patients with isolated CAE. (
