Etiopathology of psoriasis is not completely understood. Patients with psoriasis show elevated sensitivity to gluten. The aim of this study was to see the expression of celiac disease (CD)-associated antibodies gliadin IgA, gliadin IgG, and tissue transglutaminase IgA, and their correlation with HLA Cw6 in patients with psoriasis. The study comprised 56 patients with psoriasis and 60 healthy controls (HC). The levels of antibodies were detected by using ELISA technique and HLA Cw6 typing was carried out by microcytotoxicity method. HLA Cw6 was significantly expressed in psoriasis cases when compared with HC (P<0.05). CD-associated antibodies gliadin IgA/IgG and tissue transglutaminase IgA were significantly higher in the serum of patient with psoriasis when compared with HC (P<0.05, <0.05, and 0.01, respectively). Serum anti tissue transglutaminase IgA (anti tTG IgA) was significantly higher in females when compared with males and expressed more in elderly patients. There was a significant positive correlation among the antibodies (anti gliadin IgA with anti gliadin IgG: r=0.67, P<0.05; anti gliadin IgA with anti tTG IgA: r=0.45, P<0.05, anti gliadin IgG with anti tTG IgA: r=0.26, P<0.05, respectively), whereas insignificant with HLA Cw6. Our study concludes that latent CD or CD-associated antibodies were present in patients with psoriasis and also concludes that HLA Cw6 has no association with expression of these antibodies in patients with psoriasis.

StreSzczenieObecnie uważa się, że łuszczyca należy do kręgu schorzeń autoimmunologicznych, współistniejących z innymi chorobami z tej grupy. U pacjentów stwierdza się najczęściej łuszczycowe zapalenie stawów, reumatoidalne zapalenie stawów, choroby tkanki łącznej, stwardnienie rozsiane, autoimmunologiczne choroby tarczycy, celiakię, nieswoiste zapalenia jelit. Współwystępowanie tych zaburzeń może stanowić problem diagnostyczny i leczniczy (kontrowersje dotyczące stosowania kortykosteroidów). Wspólna patogeneza pozostaje nadal nie do końca wyjaśniona. Uważa się, że utrata immunotolerancji prowadzi do powstania m.in. autoreaktywnych limfocytów Th1 i Th17, które rozpoznają autoantygeny i prowadzą do ich niszczenia w danym narządzie docelowym. Pewne cechy mechanizmów immunologicznych obserwowanych w łuszczycy sugerują jej autoimmunologiczne tło. Należą do nich uwarunkowania genetyczne, np. PSORS 3 na chromosomie 4q predysponuje do celiakii, cukrzycy typu 1, choroby Gravesa-Basedowa i reumatoidalnego zapalenia stawów. Łuszczyca jest chorobą, w któ-rej decydującą rolę odgrywa aktywacja osi IL-12/Th1/IFN-γ i Th17/ IL-23. Prawdopodobnie IL-12 działa na naiwne limfocyty T i zapocząt-kowuje odpowiedź Th1, a IL-23 podtrzymuje reakcję zapalną mediowaną przez Th1, pobudza dojrzewanie i aktywność Th17 oraz wpływa na utrzymanie odpowiedniej puli komórek pamięci. Dochodzi również do zaburzenia funkcji limfocytów Treg, odpowiedzialnych za niszczenie limfocytów autoreaktywnych. Ponadto keratynocyty łuszczycowe mają zwiększoną oporność na apoptozę, której zadaniem jest eliminacja uszkodzonych komórek, aby nie zostały rozpoznane jako antygenowo obce. Badacze sugerują jednak, że początkowo następuje poliklonalna aktywacja limfocytów T wywołana przez superantygeny (m.in. paciorkowcowe białka M, peptydoglikan) lub uraz naskórka (objaw Köbne-ra), natomiast w późniejszej fazie autoreaktywne limfocyty T rozpoznają autoantygeny (keratyna 17, białka kapsydu L1 HPV 5, Pso p27) w naskórku, co prowadzi do reakcji autoimmunologicznej. Most often patients have psoriatic arthritis, rheumatoid arthritis, inflammatory bowel disease, autoimmune thyroid diseases and multiple sclerosis. Aims. The aim of the study was to investigate gastrointestinal comorbidities, identify risk factors and detect the early stages of autoimmune gastrointestinal diseases, such as Crohn's disease, ulcerative colitis and coeliac disease in patients with psoriasis. Methods. This was a hospital-based case-control study. Patients with chronic plaque psoriasis were included as cases. The control group consisted of patients with other skin diseases and who complied with the same selection criteria as cases. Two controls were selected per one case. We analysed the following antibodies (ASCA, AEP, p-ANCA, AGC, EMA, ARA, tTG, AGA) and non-specific signs of gastrointestinal diseases. Results. There were significant differences between cases and controls in several parameters. Leucocyte count, CRP, total protein, transglutaminase IgA antibodies and p-ANCA were statistically significant between groups (P < 0.05). In the binary logistic model, leucocyte count and p-ANCA (for all parameters included in the logistic model P ≤ 0.001) were associated with psoriasis. Conclusion. Patients with psoriasis should be regularly screened for coeliac and inflammatory bowel disease. Early diagnosis of gastrointestinal diseases and risk factors may prevent complications and greatly improve the patient's quality of life.

Abstract: Our results seem to imply an association between psoriasis and asymptomatic celiac disease/gluten intolerance. High percentage of positive results to guinea pig-derived tTG could be due to cellular activity of tissue transglutaminase in psoriasis.
