Abstract: Purpose We endeavored to investigate whether previous findings of an association between antemortem exposure to selective serotonin re-uptake inhibitors (SSRI) and method of suicide could be replicated. Methods Using the Swedish National Board of Forensic Medicine's toxicology database and the Swedish National Board of Health and Welfare's national registries of causes of death and prescriptions, 10,002 incidents of suicide were retrieved. Risks of violent suicide conferred by SSRIs, expressed as odds ratios (… Show more

“…Violent methods of suicide or attempts were identified in 28.4% (95% CI 7.21–49.6) of all suicidal acts (attempts + suicides), more with a diagnosis of BD-I than BD-II or MDD (Table 1) and, as expected, 2628 more with suicides than attempts, and more among men than women. The overall rate of violent acts is somewhat lower than recently reported rates that averaged 45.1% (95% CI 14.8–75.4; 36% for attempts v .…”

“…9,[20][21][22][23] Elements that may connect BD, mixed states and suicidal risk include aggressiveness and other mania-associated characteristics in both juveniles and adults. 24,25 Violent methods of suicide or attempts were identified in 28.4% (95% CI 7.21-49.6) of all suicidal acts (attempts + suicides), more with a diagnosis of BD-I than BD-II or MDD (Table 1) and, as expected, [26][27][28] more with suicides than attempts, and more among men than women. The overall rate of violent acts is somewhat lower than recently reported rates that averaged 45.1% (95% CI 14.8-75.4; 36% for attempts v. 54.6% for suicides, 38.2% among women v. 55.3% in men), probably due to regional and cultural differences.…”

“…Although poor adherence to, discontinuation of, and switches between psychotropic medications have been identified as reproducible risk factors for suicide, early discontinuation of antidepressant treatment has been reported to decrease suicide risk in the elderly [19,20]. Further, in two studies matching for suicidal propensity in Canada and Sweden, initiation of selective-serotonin-reuptake-inhibitor therapy increased the risk of violent suicide during the first month of treatment [21,22].…”

“…Additionally, although reduction in depressive symptoms is associated with reduced suicide risk (Keilp et al, 2018), the impact of SSRIs on suicidality is still a matter of debate. Specifically, SSRIs were suggested to increase the risk of suicidal ideation during treatment initiation and in certain subpopulations of depressed patients (Bielefeldt, Danborg, & Gotzsche, 2016; Bjorkenstam et al, 2013; Forsman, Masterman, Ahlner, Isacsson, & Hedstrom, 2019; Hammad, Laughren, & Racoosin, 2006; Keilp et al, 2018; KoKoAung, Cavenett, McArthur, & Aromataris, 2015; Moller, 2006; Pompili et al, 2010; Rahikainen et al, 2019; Sharma, Guski, Freund, & Gotzsche, 2016; Silverman, 2017). Thus, different treatment options for MDD and TRD are intensively studied (Bobo et al, 2016; Chen et al, 2019; Thase et al, 2018; Zhou et al, 2015).…”
