BackgroundWhen receiving palliative care, patients and their families experience altered life situations in which they must negotiate challenges in daily life, increased care and new roles. With limited time, they also experience emotional changes that relate to their uncertain future. Transitions experienced in such situations are often studied by focusing on individual aspects, which are synthesized in the following study. The aim was to conduct a qualitative meta-synthesis to explore the experiences patients and their families gain during transitions in palliative care circumstances.MethodsA qualitative meta-synthesis was conducted following an inductive approach as proposed by Sandelowski and Barroso. Inclusion criteria were studies with adult persons in palliative situations and articles published in English or German. Relevant articles were identified by researching the Pubmed and Cinahl databases, as well as by hand searches in journals and reference lists for the period 2000–2015. The findings of each study were analyzed using initial coding, followed by axial and selective coding in this order. Consequently, a conceptual model was derived from the categories.ResultsIn total 2225 articles were identified in the literature search. Finally, 14 studies were included after the selection process. The central phenomenon observed among palliative care patients and their families was maintaining normality during transitions. Transitions are initially experienced unconsciously until a crisis occurs and responsive actions are necessary, which encourages patients and families to perceive the situation consciously and develop strategies for its negotiation. Patients remain caught between hopelessness and valuing their remaining time alive. As the illness progresses, informal caregivers reprioritize and balance their roles, and after death, family members inevitably find themselves in changed roles.ConclusionsIn palliative care situations, transitions are experienced differently by patients and their families in a constant phenomenon that oscillates between unconscious and conscious perceptions of transitions. The derived conceptual model offers an additional perspective to existing models and helps to clarify the phenomenon in practical settings. The study promotes a differentiated conceptual view of transitions and emphasizes patients’ and families’ perspectives.

Zusammenfassung Einleitung und Hintergrund Die meisten Menschen möchten zu Hause sterben. Ein Großteil der Pflege im ambulanten Bereich wird von pflegenden An- und Zugehörigen geleistet. Deren Belastung und Überforderung resultieren oft in Krisensituationen und Hospitalisierungen der Patienten.Ziel der Untersuchung ist der Stand der ambulanten allgemeinen Palliativversorgung in Vorarlberg, Österreich, im Besonderen, Belastungen und Bedürfnisse pflegender An- und Zugehöriger aus Sicht der Pflege. Methode Mittels Online-Surveys wurden alle 50 ambulanten, flächendeckenden Pflegedienste Vorarlbergs eingeladen die ambulante Palliativversorgung sowie die Belastungen und Bedürfnisse pflegender Angehöriger einzuschätzen. Ergebnisse Die Rücklaufquote beläuft sich auf 76 %. Von allen Pflegekräften haben 26 % eine Palliativweiterbildung. Die Pflegedienste (in Vorarlberg: Krankenpflegevereine KPV) geben insgesamt eine hohe Belastung pflegender An- und Zugehöriger an. Eine Überforderung im Umgang mit Sterben und Tod wird am stärksten wahrgenommen (89,5 %; n = 34 KPV); fast ebenso hoch ist das Bedürfnis nach Beratung und fachlicher Sicherheit (86,8 %; n = 33); Ängste vor Krankheitsverschlechterung, Traurigkeit und Niedergeschlagenheit nehmen 84,2 % (n = 34) wahr; Überforderung bei der Pflege sowie Angst vor Krisensituationen 81,6 % (n = 31). Fast alle KPVs (94,7 %; n = 36) führen (sehr) oft Beratungs- und Begleitgespräche. Diskussion Der hohen Belastung (pflegender) An- und Zugehöriger sollte mit palliativer Kompetenz und systematisch begegnet werden. An- und Zugehörige sollten als Teil des Behandlungsteams in den Kommunikationsprozess einbezogen werden. Die interprofessionelle Zusammenarbeit sollte entsprechend den komplexen Anforderungen im ambulanten palliativen Setting mehr forciert und standardisiert werden.

Objetivo: describir la carga de cuidado de los cuidadores principales de personas con enfermedad crónica pertenecientes a un programa de hospitalización en casa de la ciudad de Manizales (Colombia). Materiales y métodos: estudio cuantitativo y observacional de corte transversal, cuya muestra estuvo constituida por 76 cuidadores principales de personas con enfermedad crónica que recibían servicios de salud por medio de un programa de atención domiciliaria. Para la recolección de los datos se aplicó el cuestionario de Zarit. Resultados: el 85,5% de los cuidadores eran mujeres. El 34,6% tenía más de 60 años, el 36,8% eran casados y con nivel de escolaridad técnico, tecnólogo o universitario en el 57,9%. El cuestionario de Zarit reportó sobre carga leve para el 22,4% y sobrecarga intensa en el 21,1%. En este grupo de cuidadores las subescalas que mostraron mayores promedios se relacionan con, el impacto del cuidado, la carga interpersonal y la indecisión sobre el cuidado. Solo se encontró asociación leve entre el número de horas diarias dedicadas al cuidado y la carga del cuidado (p=0,033), mostrando nivel de sobrecarga intensa en aquellos que dedicaban más de 15 horas diarias al cuidado. Conclusiones: el 43,5% de los cuidadores presentaba algún nivel de sobrecarga. Para estos cuidadores los mayores promedios se ubicaron en las subescalas relacionadas con el impacto del cuidado, carga interpersonal e indecisión sobre el cuidado. Es importante realizar otros estudios en los que se pueda analizar los predictores multidimensionales que generan la sobrecarga.
