Abstract: Background The outcome of cardiogenic shock complicating myocardial infarction has not appreciably changed in the last 30 years despite the development of various percutaneous mechanical circulatory support options. It is clear that there are varying degrees of cardiogenic shock but there is no robust classification scheme to categorize this disease state. Methods A multidisciplinary group of experts convened by the Society for Cardiovascular Angiography and Interventions was assembled to derive a proposed cla… Show more

“…4 9 Takotsubo syndrome heart failure and shock management algorithm. SCAI shock stages adapted from Baran et al [136] and Jentzer et al [137]. AHF Acute heart failure, BiV biventricular, CI cardiac index, CPO cardiac power output, ECPELLA VA-ECMO combined with Impella, ECPR extracorporeal cardiopulmonary resuscitation, LV left ventricle, LVEDP left ventricular end-diastolic pressure, LVOTO left ventricular outflow tract obstruction, MCS mechanical circulatory support, MR mitral regurgitation, PAPI pulmonary artery pulsatility index, PCWP pulmonary capillary wedge pressure, SCAI Society for Cardiovascular Angiography and Interventions, TS takotsubo syndrome, VA-ECMO veno-arterial extracorporeal membrane oxygenation Long-term therapy TS was initially described as a "stress cardiomyopathy" with an emotional trigger in early years.…”

“…In 2005, the observational CRUSADE registry study was the rst to demonstrate that intravenous morphine administration in patients presenting with NSTE-ACS was statistically associated with a higher rate of adverse outcomes (3.80% vs 3.00% for MI, 3.80% vs 2.30% for cardiogenic shock, 5.50% vs 4.70% for in-hospital death, and 8.50% vs 7.10% for composite of death and MI) [13]. Zaza et al found that intravenous morphine was associated with inhospital MACEs and 30-day mortality and most strongly associated with admission Killip class III-IV (OR=2.3, 1.5-3.5) [9]. According to the studies described above, morphine was not associated with signi cant adverse outcomes in AMI without HF, but the opposite result was observed in AMI patients with HF.…”

“…The primary endpoint was in-hospital complications including a composite of MACEs (Cardiac death +recurrent MI + unstable angina + cardiogenic shock), cardiac death, recurrent MI, unstable angina, and cardiogenic shock [9]. The secondary endpoint was the requirement of in-hospital invasive mechanical ventilation.…”

“…Die Schockschwere kann in der Akutphase anhand der SCAI-Klassifikation (A: Risiko für sich entwickelnden Schock, sog. Präschock, bis hin zu E: Extremsituation mit fortlaufender CPR) festgelegt werden [5]. Im vereinfachten Schock-Score wird ein Versagen von Herz, Nieren und Leber (sog.…”
