“…The same is true for induced sarcoidosis, which may be limited to 1 organ [4, 7, 8, 9, 10, 11]. For example, it has been reported to present very atypically and involve just the hypopharynx [13]. …”

“…Those observations led them to raise the possibility that the -857 genotype of the TNFa promoter may affect its expression at the protein level in the absence of other confounding factors, such as the possible effect of AS on TNFa level, and they also concluded that the impact of -857T allele on TNFa expression does exist (21). Tong et al reported that -857C allele showed better responsiveness than -857T allele to the TNF inhibitors and the -857C/C genotype is predictor of good response in spondyloarthritis, inflammatory bowel disease and ankylosing spondylitis patients (23,24). Vasilopoulos et al (25) also reported that carriage of -857C allele was associated with positive response to pharmacotherapy in patients treated with etanercept which is one of the TNF inhibitors.…”

“…While there have been multiple studies outlining the positive effects of the therapy, a recent study outlines a potential complication. A patient treated with adalimumab for psoriatic arthritis went on to develop sarcoidosis of the hypopharynx [32]. This has been the first reported case of sarcoidosis of the hypopharynx, and demonstrated a complex clinical scenario in that discontinuation of the drug resulted in resolution of the sarcoidosis.…”

“…This has been the first reported case of sarcoidosis of the hypopharynx, and demonstrated a complex clinical scenario in that discontinuation of the drug resulted in resolution of the sarcoidosis. This is an unforeseen complication of TNF-a therapy resulting in an extremely rare manifestation of sarcoidosis [32].…”
