Rupture of pulmonary artery induced by Swan-Ganz catheter: success of coil embolization

Abstract: A 75-year-old woman suffered severe haemoptysis after insertion of a Swan-Ganz catheter in an intensive care unit. Control of the pulmonary artery haemorrhage was quickly and successfully achieved with stainless steel coils as the embolic material.

“…As expected when using contrast agents during angiography, some degree of acute renal insufficiency occurred [37]. Among other complications, minor pulmonary infarctions were common following coil embolization but usually did not significantly affect the patient's clinical status or arterial blood gases [39,40]. The embolized coils occasionally migrated distally or protruded from the vessel, which has been reported to be nonfatal [39].…”

Aetiologies include iatrogenic in 16 % (6 cases) [ [46] , [47] , [48] , [49] , [50] ], valvular heart disease in 14 % (5 cases) [ [51] , [52] , [53] ], trauma in 11 % (4 cases) [ 54 , 55 ], cancer in 11 % (4 cases) [ 56 , 57 ], infectious disease in 5 % (2 cases) [ 4 , 58 ], and other more rare complication of conditions, as pregnancy [ 59 ], Takayasu's arteritis [ 60 ], idiopathic pulmonary artery dilatation [ 61 ], amyloidosis, hypergammaglobulinemia of unknown cause and obesity [ 4 ], and others [ [62] , [63] , [64] , [65] , [66] ]. The diversity of the underlying aetiology suggests that triggers other than pulmonary hypertension may also lead to pulmonary artery degeneration and result in PAD.…”

“…Additionally, embolization is a procedure that is typically performed in a remote location requiring transport of a potentially unstable patient who should be monitored in an intensive care unit. Lastly, there are several complications associated with coil embolization of vessels, including infarctions, the exact etiology being treated in this scenario [ 13 , 14 ].…”
