Background. Reciproc instruments are the only contemporary root canal instruments where glide path preparation is no longer strictly demanded by the manufacturer. As the complete preparation of root canals is associated with success in endodontic treatment we wanted to assess the ability and find predictors for Reciproc instruments to reach full working length (RFWL) in root canals of maxillary molars in primary root canal treatment (1°RCTx) and retreatment (2°RCTx) cases. Methods. This retrospective study evaluated 255 endodontic treatment cases of maxillary molars. 180 were 1°RCTx and 75 2°RCTx. All root canals were prepared with Reciproc instruments. The groups were compared and in a binary logistic regression model predictors for RFWL were evaluated. Results. A total of 923 root canals were treated with Reciproc without glide path preparation. This was possible in 885 cases (95.9%). In 1°RCTx cases 625 of 647 (96.6%) canals were RFWL and in 2°RCTx cases 260 of 276 (94.2%). In second and third mesiobuccal canals (MB2/3) 94 out of 106 (88.7%) were RFWL with Reciproc in 1°RCTx and in the 2°RCTx treatment group 50 out of 52 cases (96.2%). In mesiobuccal (MB1) canals "2°RCTx" was identified as negative predictor for RFWL (OR 0.24 (CI 0.08-0.77)). In MB2/3 canals full working length was reached less often (OR 0.04 (CI 0.01-0.31)) if the tooth was obliterated and more often if MB2/3 and MB1 canals were convergent (OR 4.60 (CI 1.07-19.61)). Discussion. With Reciproc instruments the vast majority of root canals in primary treatment and retreatment cases can be prepared without glide path preparation.

To compare the efficacy of Reciproc (reciprocating) and MTwo (rotary) in the filling material removal during endodontic retreatment. Methods: Thirty simulated root canals (plastic blocks) were prepared and filled. The blocks were divided into 3 groups according to the filling removal techniques: Group I-hand files and Gates Glidden burs; Group II-Rotary technique with second series files of Mtwo system; Group III-Reciprocating technique with instrument R40 Reciproc. The blocks were radiographed, the total area of the root canal and the amount of remaining plug material was calculated. The total time required to complete the procedure was recorded and the instruments were evaluated for fracture or deformation. Results: The mean percentage of obturator material remaining in the root canal wall was 15.36% in Group I, 11.56% in Group II and 10.36% in Group III. There was a statistical difference between Group 1 and the other Groups. There was no statistical difference between Groups II and III. Removal of filling material was significantly faster in Group III (437.433 s), followed by Group II (616.535 s) and Group I (1587.651 s). Conclusion: The best results of filling material removal were obtained by groups II and III without statistical difference between them. The reciprocating technique was the fastest among the techniques tested, followed by the rotary technique and manual technique files.

Objective:The purpose of this study was to assess the role of cross section on cyclic fatigue resistance of One Shape, Revo-S SU, and Mtwo rotary files in continuous rotation and reciprocating motion in dynamic testing model.Materials and Methods:A total of 90 new rotary One Shape, Revo-S SU, and Mtwo files (ISO size 25, taper 0.06, length 25 mm) were subjected to continuous rotation or reciprocating motion. A cyclic fatigue testing device was fabricated with 60° angle of curvature and 5 mm radius. The dynamic testing of these files was performed using an electric motor which permitted the reproduction of pecking motion. All instruments were rotated or reciprocated until fracture occurred. The time taken for each instrument to fracture was recorded. All the fractured files were analyzed under a scanning electron microscope (SEM) to detect the mode of fracture. Statistical analysis was performed using one-way ANOVA, followed by Tukey's honestly significant difference post hoc test.Results:The time taken for instruments in reciprocating motion to fail under cyclic loading was significantly longer when compared with groups in continuous rotary motion. There was a statistically significant difference between Mtwo rotary and the other two groups in both continuous and reciprocating motion. One Shape rotary files recorded significantly longer duration to fracture resistance when compared with Revo-S SU files in both continuous and reciprocating motion. SEM observations showed that the instruments of all groups had undergone a ductile mode of fracture.Conclusion:Reciprocating motion improved the cyclic fatigue resistance of all tested groups.
