Role of Endourological Procedures (PCNL and URS) on Renal Function: a Systematic Review

Abstract: Purpose of Review To present the latest evidence related to the impact of ureteroscopy (URS) and percutaneous nephrolithotomy (PCNL) on the renal function. Recent Findings Our review suggests that the overall renal function is not detrimentally affected by endourological interventions (URS, PCNL). This is however influenced by the preoperative renal function, presence of comorbidities such as diabetes and hypertension. For PCNL procedures, tract multiplicity, preoperative UTI, and postoperative bleeding also … Show more

“…The choice of treatment depends on stone factors, patient factors including comorbidities, surgical expertise, and also underlying renal function. A systematic review by Reeves et al suggests that the overall renal function is not always detrimentally affected by endourological interventions [2]. Morbidities after PNL such as fever, bleeding, pleural or visceral injury and significant nephron loss have been well described [11].…”

“…Effect on renal function is influenced by preoperative renal status and presence of comorbidities such as diabetes mellitus and hypertension [15]. Violation of the renal parenchyma, high irrigation pressure, tract multiplicity, preoperative urine infection and postoperative bleeding are reported as attributes causing post-PNL AKI [2,16]. However, subsequent improvement in renal function is seen in almost all renal units that are obstructed and infected [15,17].…”

“…KSD is an important risk factor for renal dysfunction which could have important implications for further management options post-operatively [33]. This is important for IBD and cancer patients where a wide range of medical therapies including chemotherapy are available that patients with chronic kidney disease (CKD) may not be able to access [17•].…”

“…tea, spinach, bananas) and increased fluid intake. Oral calcium supplements may be considered as an option because they bind with enteric oxalate, reducing the amount of free oxalate being absorbed and excreted renally [33]. There are medications to help reduce urinary calcium excretion and so increase enteric calcium concentrations and reduce enteric free oxalate concentrations.…”
