Risk Factors for Delayed Presentation Among Patients with Musculoskeletal Injuries in Malawi

Abstract: Background:The burden of injuries is high in low-income and middle-income countries such as Malawi, where access to musculoskeletal trauma care is limited. Delayed treatment can worsen trauma-related disability. Understanding risk factors for delayed hospital presentation will assist in guiding trauma system development. Methods:We examined the records of 1,380 pediatric and adult patients with fractures who presented to the orthopaedic clinics of 2 urban referral hospitals and 2 rural district hospitals in Ma… Show more

“…Injuries to extremities due to tra c accidents and prolonged denervation result in soft tissue loss and are considered the most frequent clinical problem [1,2,4]. In recent years, stem cell transplantation therapy has been considered an effective strategy for the regeneration of damaged tissues.…”

“…Severe trauma due to tra c accidents, severe injuries, and long-term denervation injury resulting in soft tissue loss is considered one of the most frequent clinical problems, and surgical reconstruction does not fully repair the tissue and frequently causes donor site morbidity [1][2][3][4]. Therefore, new treatment methods must be explored for severe skeletal muscle injuries [5][6][7].…”

“…Fluctuations in disease burden in Malawi have been reported [45], and may be associated with seasonal variations in resource availability. Though the reason remains unclear, fewer patients with acute fractures present to orthopaedic clinics during the dry season [46]. We may have therefore overestimated capacity, compared to during the rainy season when higher trauma burden may result in greater resource utilization.…”

“…This may be due to relatively de cient transportation infrastructure in the central region, perceived poor quality of care or long wait times at KCH, or increased utilization of non-governmental hospitals by patients. 37 Thus, we focused our study on the experiences of patients who had been admitted to KCH.…”

“…55,56 This study's ndings must be interpreted with caution, recognizing that our patient population was not a representative sample of all Malawian patients with FSFs. We chose to focus on care delivery at Kamuzu Central Hospital, given the relatively high rates of delayed presentation previously observed there, 37 in an effort to examine challenges where they were perhaps the greatest. This was a qualitative study, undertaken to generate hypotheses.…”
