Schwannoma is a rare slow-growing neurogenic tumor of the nerve sheath origin. They are commonly seen in middle-aged with a female to male ratio of 1:1.2. Here we present three cases of retroperitoneal schwannoma in our urology department. In two cases, we decided surgery management and in one case, we used watchful waiting. After a 3-year follow-up, case 1 was asymptomatic without changes in the lesion. They are commonly seen in middle-aged with a female to male ratio of 1:1.2. Visceral schwannomas are infrequent. Computerized tomography (CT) images present areas of cystic degeneration, calcification or hemorrhage. Extension of the mass along the nerve may indicate the neural origin of the mass. It is important to exclude a malignant retroperitoneal tumor to choose the correct treatment.Here we present three cases of retroperitoneal schwannoma in our urology department. Case Reports Case 1A 54-year-old female smoker presented in our department with back pain on the right. Abdominal ultrasound and CT with contrast revealed a 4.2 × 3.2 cm mass located in the retrocaval area and medial to the right kidney (Fig. 1). The findings of the images were compatible with pheochromocytoma or paraganglioma. Biochemical workup included plasma and urine catecholamine. All studies were normal. The imagingguided biopsy was done. Microscopic findings showed a mass composed of fusiform cells with abundant collagen and spaces with macrophages. No mitoses were observed. Immunohistochemistry showed that the tumor cells were positive for S-100, BCL2, CD56, CD99 and negative for CD34 and muscle specific actin and desmin. The Ki67 activity was 1%. These findings were consistent with benign schwannoma.Because it was a benign case with small mass and the patient was asymptomatic, we decided watchful waiting with radiological periodic controls. Schwannomas are benign sheat tumors composed of Schwann cells, which are covering peripheral nervs. Althought schwannomas can be found in any localisation, retroperitoneal scheannomas are extremly rare. Multiple schwannomas (neurilemomas) can be part of neurofibromatosis type II, as well as schwannomatosis. Depending on tumor localization, disease symptoms differ. We present a patient with multiple spinal as well as peripheral schwannomas as a part of schwannomatosis. Patient was successfully operated in cooperation between general surgeon and neurosurgeon. ApstraktŠvanomi su tumori porekla Švanovih ćelija, koje učestviju u građi omotača perifernih nerava. Iako se švanomi mogu naći na bilo kojoj lokalizaciji, u retroperitonealnom prostoru su izuzetno retki. Multipli švanomi (raniji naziv: neurilemomi) mogu se naći u sklopu neurofibromatoze tip II, kao i u sklopu švanomatoze. U zavisnosti od lokalizacije švanoma, simtpomatologija je različita. Mi prikazujemo pacijenta sa multiplim spinalnim, ali i perifernim švanomima u sklopu švanomatoze, koji je u više navrata uspešno operisan multidisciplinarnim pristupom i saradnjom opšteg hirurga i neurohirurga. Ključne reči Švanomi; Neurofibromatoza UvodŠvanomi predstavljaju jednu od najčešćih solitarnih lezija 1 . Međutim, mogu se javiti i kao multiple lezije u sklopu bolesti kao što su neurofibromatoza i švanomatoza 2 , koje se karakterišu multiplim švanomima 3 . Za razliku od švanomatoze, kod neurofibromatoze tip II pored multiplih perifernih švanoma i spinalnih švanoma, javljaju se i bilateralni švanomi vestibularnih nerava. Takođe, u odnosu na švanomatozu, neurofibromatoza tip II predstavlja ozbiljno, progresivno oboljenje, koje neretko ugrožava život pacijenta 4 . Bol je čest simptom kod švanomatoze, dok se motorna slabost retko javlja 5 .Mi prikazujemo pacijenta sa multiplim spinalnim, ali i perifernim švanomima, koji je u više navrata uspešno operisan multidisciplinarnim pristupom i saradnjom opšteg hirurga i neurohirurga. Kod pacijenta se pored bola javila i motorna slabost mišića pokretača desnog stopala. Prikaz slučajaPrikazujemo pacijenta starosne dobi 47 godina koji se prvi put javio lekaru u svojoj 37 godini zbog bolova u donjem delu leđa, kao i duž spoljašnje strane desne butine. Nakon urađene magnetne rezonance torakalne i lumbo-sakralne kičme na kojoj je viđena tumorska formacija benignih karakteristika u nivou Th12 do L1 pacijent je upućen neurohirurgu, te je ubrzo operisan i odstranjena promena je patohistološki verifikovana kao švanom. Schwannomas usually occur in adults being between the second and fifth decades, and such neoplasms are extremely rare in a pediatric population. In addition, they are not normally found in the retroperitoneal region. Here, we present a pediatric case of a retroperitoneal schwannoma in an adrenal location where the tumor was not able to be preoperatively differentiated from other benign or malign adrenal gland tumors. In our opinion, this tumor can be included in the differential diagnosis of a nonfunctioning retroperitoneal adrenal mass in children.
