Retinal detachment after phacoemulsification in high myopia: Analysis of 2356 cases

Abstract: The risk for postoperative RD in this study (1.5% to 2.2%) corresponds to the incidence of pseudophakic RD and idiopathic RD in myopia described in the literature. The risk profile for postoperative RD in this series cannot be distinguished from the risk profile for idiopathic RD in myopia of this extent. The true effect of prophylactic treatment of degenerative lesions remains to be elucidated.

“…9 Cataract surgery in highly myopic eyes can be complicated by zonular instability, extreme deepening of the anterior chamber (known as "lens-iris diaphragm retropulsion syndrome"), higher risk of postoperative retinal detachment, difficulties in predicting correct IOL power, and lower visual potential. [10][11][12] Theoretically, some of these challenges may be avoided by using FLACS, which helps to create a perfectly round anterior capsulotomy and softens the lens nucleus.…”

“…Der Anteil mit gittriger Degeneration steigt mit der Achslänge (bis über 30 mm) auf bis zu 15 % der untersuchten Augen [72]. Cambiaggi [13] [107]. Dabei erhöht bereits allein die Myopie die jährliche Inzidenz (10 auf 100.000), eine Netzhautablösung zu erleiden [97].…”

“…PCO is caused by residual lens epithelial cells, which are left behind after cataract surgery. These residual lens epithelial cells have been shown to produce interleukins (36)(37)(38), fibroblast growth factor and transforming growth factor beta in inoculation media. Cytokines may also play a role in the proliferation and development of lens epithelial cells; therefore inhibition of cytokines may lead to prevention of PCO (39).…”
