Patients with hepatic metastases of melanoma have a very poor prognosis, with a median overall survival of less than 6 months. There are several small heterogeneous studies that have shown an association with prolonged survival in those patients treated with hepatic resection, but the role of surgery remains unclear. We evaluated the safety and efficacy of hepatic resection in a population-based study in the Netherlands for patients with metastatic melanoma and assessed the factors that could affect disease-free and overall survival. Patients with hepatic melanoma metastases who underwent potentially curative resection were identified between 1994 until 2010 using the PALGA database, a nation-wide network and registry of histopathology and cytopathology in the Netherlands. They were retrospectively evaluated for clinical and pathological factors with respect to recurrence and survival using Kaplan-Meier curves to assess survival and univariate regression analysis for the assessment of potential prognostic factors. A total of 32 patients were identified in 15 hospitals, 19 men and 13 women. The median age of the patients at the time of hepatic resection was 52 years (range 27-69). Postoperative complications occurred in five patients (15%), without postoperative mortality. The median follow-up was 21 months (range 3-65). The median disease-free survival was 11 months (range 0-57) and the median overall survival was 29 months (range 4-66). Significant prognostic factors for overall survival in univariate analysis were the distribution and number of metastases, as well as the type of hepatic resection (major or minor). Hepatic resection in patients with resectable metastatic melanoma is safe and might be associated with a prolonged survival in a highly selected group of patients.

Liver-directed treatments for liver metastasis from gastric adenocarcinoma: comparison between liver resection and radiofrequency ablation

Background Although various liver-directed treatment modalities, such as liver resection and radiofrequency ablation (RFA), have been applied to treat liver metastases from gastric cancer, optimal management of them remains controversial. In patients with liver metastasis from gastric cancer, we investigated the short-and long-term outcomes of liver resection and RFA and analyzed factors influencing survival. Methods A total of 98 gastric cancer patients with liver metastasis and no extrahepatic disease were treated by liver resection (n = 68) or RFA (n = 30). Short-and long-term outcomes were evaluated retrospectively for each of the liver-directed treatments. Results Severe complication rates did not differ between liver resection (18 %) and RFA (10 %) (p = 0.333). Only one treatment-related mortality occurred in the liver resection group. No statistically significant difference in survival was noted between the treatment groups. Median overall survival after liver resection was 24 months, with 3-year overall and progression-free survival rates of 40.6 % and 30.4 %, respectively. Median overall survival after RFA was 23 months, with 3-year overall and progression-free survival rates of 43.0 % and 37.4 %, respectively. Only the size of the metastases was shown to be an independent prognostic factor for gastric cancer patients with liver metastasis. Conclusions In select patients with liver metastasis from gastric cancer, liver resection and RFA showed satisfactory and comparable short-and long-term results. Thus, systemic chemotherapy may not be the only therapeutic option for patients with liver metastasis, and possible liver-directed treatment options for such patients should be considered on an individual basis.

INTRODUÇÃOAtualmente, a ressecção cirúrgica de metástases hepáticas de etiologia colorretal é a única modalidade com intenção curativa ou de prolongamento da sobrevida. Em séries mais recentemente publicadas, a sobrevida dos pacientes submetidos à sua ressecção completa (R0) tem variado de 25 % até 55 % em cinco anos (1-3). Em contrapartida, quando indivíduos com disseminação hepática são submetidos a tratamento quimioterápico exclusivo, raramente alcançam mais do que três anos de sobrevida (4).
