BACKGROUND: Although bariatric surgery is highly effective for the treatment of obesity and its comorbidities, preoperative weight loss has an impact on its results. AIMS: The aim of this study was to correlate preoperative weight loss with the outcome of bariatric surgery using the Bariatric Analysis and Reporting Outcome System scores. METHODS: This is a cross-sectional, observational study with 43 patients undergoing bariatric surgery that compared a group of 25 patients with a percentage of preoperative excess weight loss ³8% with a group of 18 patients with a percentage of preoperative excess weight loss <8% or with weight gain. The research took place at the bariatric surgery outpatient clinic of the Oswaldo Cruz University Hospital with patients 1 year after the surgery. RESULTS: Patients had a mean age of 40.8 years (42.7 percentage of preoperative excess weight loss ≥8% vs. 38.2 percentage of preoperative excess weight loss <8%, p=0.095). No significant difference was found between the two groups regarding preoperative comorbidities and body mass index at entry into the program. Higher preoperative body mass index (48.69 vs. 44.0; p=0.029) was observed in the group with percentage of preoperative excess weight loss <8%. No significant difference was found regarding the percentage of excess weight loss (71.4±15.4%; percentage of preoperative excess weight loss ≥8% vs. 69.47%±14.5 percentage of preoperative excess weight loss <8%; p=0.671), the result of the surgery according to the Bariatric Analysis and Reporting Outcome System scores protocol, the resolution of comorbidities, the quality of life, and the surgical complications between the two groups. CONCLUSIONS: Based on the available data, it is reasonable that bariatric surgery should not be denied to people who have not achieved pre-established weight loss before surgery.

Objective: to evaluate evidence on risk factors for the development of surgical site infection in bariatric surgery. Method: integrative review. The search for primary studies was performed in four databases. The sample consisted of 11 surveys. The methodological quality of the included studies was assessed using tools proposed by the Joanna Briggs Institute. Data analysis and synthesis were performed in a descriptive manner. Results: surgical site infection rates ranged from 0.4% to 7.6%, considering the results of primary studies, in which patients underwent laparoscopic surgery. In surveys of participants undergoing surgical procedures with different approaches (open, laparoscopic or robotic), infection rates ranged from 0.9% to 12%. Regarding the risk factors for the development of this type of infection, antibiotic prophylaxis, female sex, high Body Mass Index and perioperative hyperglycemia are highlighted. Conclusion: conducting the integrative review generated a body of evidence that reinforces the importance of implementing effective measures for the prevention and control of surgical site infection, by health professionals, after bariatric surgery, promoting improved care and patient safety in the perioperative period.

Modificações fisiológicas podem ser observadas no paciente pós bariátrica, uma dessas modificações remete a deficiências nutricionais, em sua maioria decorrente do método cirúrgico utilizado, também referente ao impacto que esta cirurgia pode trazer, onde a ausência de nutrientes por alimentação ou suplementação levam a impactos maiores à saúde desses pacientes que muitas vezes se faz necessário. Esses déficits de nutrientes, quando não tratados, podem agravar ou gerar diversas patologias, como anemia ferropriva, anemia megaloblástica, doenças ósseas entre outras. O estudo objetiva analisar e compreender as diretrizes da cirurgia bariátrica, evidenciando as carências que podem ocorrer após o procedimento e investigando também, as estratégias de suplementação que podem ser usadas. Trata-se de uma revisão bibliográfica narrativa, onde a coleta de dados se deu através das bases de dados Pubmed, Scielo e a Associação Brasileira para o Estudo da Obesidade e Síndrome Metabólica. Foram incluídos artigos datados nos últimos 5 anos, além de informações coletadas no Departamento de Gastroenterologia da Faculdade de Medicina de São Paulo (2014) e Arquivos Brasileiros de Cirurgias Digestivas (2015). Utilizando os seguintes DeCS: bariátrica; obesidade; obesidade mórbida; hipovitaminoses; micronutrientes; deficiências nutricionais, nos idiomas inglês, português e espanhol. Dessa forma, se faz necessário suplementar micronutrientes para prevenir e tratar as possíveis inadequações nutricionais do paciente pós bariátrica.
