The Effect of Guided Imagery on Death Anxiety, Empathy and Attitude towards Terminally Ill Patients in Oncology Nurses

Objective: Enabling oncology nurses to become aware of their own mortality is important to provide a dignity and quality end of life care. The aim of this study is to evaluate the effect of guided imagery on death anxiety, empathy and attitude towards terminally ill patients in oncology nurses. Guided imagery may be an effective tool to help nurses at the end of life care. Methods: This quasi-experimental research included 43 oncology nurses who were chosen using total population sampling method. The data was collected by using the nurse information form, the “Thorson-Powell Death Anxiety Scale”, the “Approach to Death and Dying Patients Attitude Scale”, the “Interpersonal Reactivity Index” Results: The nurses in the intervention and control groups were similar in terms of characteristics (P> .05). In intragroup comparison of the experimental group, it was determined that while both death anxiety and empathy levels increased, scores of avoiding terminally ill patients of nurses also decreased (P< .05). On the other hand, in the intergroup comparison, it did not affect death anxiety and attitude towards the terminally ill patients (P> .05); however, it was found that it increased empathy levels of nurses (P< .05). Conclusion: Guided Imagery may be an effective technique in oncology nurses to improve positive attitude towards the terminally ill patients and increase empathy level. Using guided imagery can facilitate clinical nursing practice by changing nurses’ perspective towards the nearing people end of life. However, further experimental research are needed to validate this study.

Günümüzde bireyler, hayatın birçok noktasında ruhsal ve fizyolojik yansımalara sebebiyet veren travmatik deneyimler yaşayabilmektedirler. Travma, kişilerin doğal olan ve doğal olmayan biçimde karşılaşabilecekleri olgular çerçevesidir. Kişini sel, deprem, heyelan gibi doğal felaketler, cinayet, istismar, savaş, terör eylemleri, beklenmedik güç hastalıklar gibi akış içindeki hayatta kişinin beklemediği anda baş edebilme gücünü zorlayan ve devam edebilmesini imkânsız kılabilen deneyimler sürecidir. Travmatik deneyimler hayatın her döneminde kişilerin karşısına çıkabilmektedir. Birey, benlik algı çerçevesi gelişiminden itibaren travmatik deneyimler yaşayabilir ve izlerini fizyolojik ve ruhsal bağlamda hayat sonuna kadar yanında taşıyabilmektedir. Bu deneyimler direkt kişisel veya bir yakını dolayısı ile maruz kalınabilir . Bireylerin yaşadığı travmatik deneyimler, kişiler üzerinde fizyolojik; kardiyovasküler sistem bozuklukları, solunum sistemi, kas sistemi ve sinir sistemi dahil olmak üzere birçok noktayı tetikleyebilirken (Solomon & Heide, 2005), ruhsal olarak ise kronikleşen kaygı, stres, duygu ve durum kontrol güçlükleri, endişe ve panik hali tetiklenerek travma sonrası stres bozukluğu tanımlanabilmektedir (Isobel et al., 2017) TRAVMA NEDİR? Kişinin doğumdan itibaren başına gelebilen, tanık olabildiği veya dolaylı olarak öğrendiği duygusal ve fiziksel sağlığını olumsuz olarak etkileyen, stres faktörleri ile tetiklenerek meydana gelen durumlar bütünüdür.Travma kelimesi antik Yunan çağına dayanarak 'yara' ve 'hasar' şeklinde ifadesi sağlanmıştır (Winders et al., 2020). DSM 5 ruh sağlığı el kitabında travmatik olay; ''doğrudan yaşanılan veya doğrudan tanıklık edilen ya da aile bireyi, yakın bir arkadaşın başına gelen ya da mesleki olarak deneyimlenen gerçek göz korkutucu bir biçimde ölümle ya da ağır yaralanmayla karşı karşıya gelmiş ya da cinsel saldırıya uğramış olma'' şeklinde açıklanmıştır (APA, 2022).Travmatik deneyimler sonucunda kişiler işlevselliklerini ve yaşamın rutin uyumundan kopmalar yaşayabilmektedir. Travmatik ögeler çeşitlere sahiptir, bunlar ''kişinin gelişimsel krizleri; ergenlik dönemi, okuldan mezun olmak, kariyer veya iş değiştirmek, evlenmek, çocuk sahibi olmak, duruma bağlı krizler; ayrılmak ve boşanmak, işini kaybetmek, trafik kazası yapmak, ciddi bir hastalık yaşamak, ani ölümler, ekonomik kriz, kompleks gelişen krizler;

Purpose: Long-lasting COVID-19 symptoms are closely associated with psychiatric sequelae. However, little is known about whether it is possible to predict a protracted course early in the disease. In this study, we aimed to understand how post-COVID-19 psychiatric symptomatology evolves over time and the variables that affect these symptoms. Materials and Methods: Anxiety, depression, acute stress, and sleep disorders among patients with COVID-19 were prospectively evaluated during hospitalization and six months after discharge. Adult patients recently admitted to non-intensive care units with COVID-19 were eligible. Their psychiatric status was assessed using the Hospital Anxiety and Depression Scale, National Stressful Events Survey Acute Stress Disorder Short Scale, Pittsburgh Sleep Quality Index. The patients’ clinical data were gathered from hospital records. Six months after they were discharged, the same questionnaire and a checklist to assess ongoing physical symptoms were distributed to the patients via WhatsApp. Results: 143 participants were enrolled, 47 of whom completed the study. The results showed that patients’ depression, anxiety, and insomnia significantly decreased during the follow-up. However, there was an increase in the number of patients who reported acute stress symptoms. The highest C-reactive protein level during acute infection (OR=1.09) predicted depression during the follow-up. Experiencing a higher number of infectious symptoms during hospitalization predicted a higher number of protracted symptoms six months later (OR=1.5). Conclusion: This indicates that the severity of systemic inflammation during acute COVID-19 infection may predispose patients to persistent depression. Patients with a higher number of symptoms during acute infection may be at risk of developing long-term COVID-19.
