Background: Chronic Low Back Pain (CLBP) significantly impacts patients' quality of life and is often associated with kinesiophobia, the fear of movement due to pain. The Tampa Scale for Kinesiophobia (TSK) is a recognized tool for assessing this condition, but there is a need for a version tailored to Urdu-speaking populations. Objective: This study aimed to translate and validate the TSK into Urdu for patients with CLBP, ensuring its reliability and cultural relevance in the Pakistani context. Methods: This descriptive cross-cultural linguistic validation study followed Bombardier, Beaton, and Guillemin’s guidelines. The TSK underwent forward and backward translation by two independent translators, a professor of Urdu literature and a medical professional. A final version was created and reviewed by bilingual experts, then validated by a panel of 20 physical therapists. This validated version was tested on 150 patients to determine the reliability of the Urdu version of the instrument. Results: The Urdu version of the TSK achieved a Cronbach’s alpha of 0.986 post-translation and 0.817 after testing with the target population. The inter-item correlation varied between 0.443 to 0.987. The intra-class correlation coefficient, indicating test-retest reliability, ranged from 0.1 to 0.8, demonstrating high reliability. Conclusion: The Urdu version of the TSK is a valid and reliable tool for assessing kinesiophobia in patients with CLBP. It exhibits good construct validity, high internal consistency, as well as good test-retest reliability and low to moderate responsiveness.

Background Patellofemoral pain syndrome (PFPS) is a common musculoskeletal condition in young and active adults. Exercise therapy is an essential part of rehabilitation in people with PFPS (PwPFPS). Telerehabilitation is an innovative treatment approach that has been used in several musculoskeletal conditions. This study aims to investigate the non-inferiority of telerehabilitation through a smartphone application, the Vito App, compared to face-to-face physical therapy on reducing pain and improving physical function, quality of life, and psychological factors. Methods This randomized controlled trial will include 60 PwPFPS. to a control group (face-to-face physical therapy) or an experimental group (telerehabilitation). The intervention for both groups consists of stretching, strengthening, balance, and functional exercises for 6 weeks and three sessions per week. The primary outcomes are pain intensity by visual analog scale (VAS), physical function by the Kujala questionnaire and functional tests including the bilateral squat, anteromedial lunge, and step down, and quality of life by the Knee Injury and Osteoarthritis Outcome Score (KOOS) questionnaire quality of life subscale. Secondary outcomes are psychological factors such as anxiety and depression assessed with the Hospital Anxiety and Depression Scale (HADS) questionnaire, kinesiophobia assessed with the Tampa scale, and pain catastrophizing assessed with the Pain Catastrophizing Scale (PCS). Assessments will be held in 3 phases: pre-test (before the intervention), post-test (after the 6 weeks of intervention), and follow-up (1 month after the end of the intervention). Discussion We expect that both the control group and experimental group will show similar improvements in clinical and psychological outcome measures. If our hypothesis becomes true, PwPFPS can use telerehabilitation as a practical treatment approach. Telerehabilitation can also enhance accessibility to rehabilitation services for active adults and for people living in remote and rural areas. Introduction The global rise in lower-limb amputations is mainly due to diabetes and vascular complications. Amputations cause serious physical and psycho-social disabilities which impair locomotion and compromise patients quality of life. Biopsychosocial factors such as altered gait parameters, poor prosthetic fit, depression, fear avoidance behaviours and stigma increase the risk of individuals incurring lower back pain; the leading cause of secondary disability amongst persons with lower-limb amputation. Exploring the potential mechanistic pathways of lower back pain development is important to inform timely patient-centred programmes of care. Currently, limited information exists to inform the design of such programmes. Thus, there is a pressing need to understand the bio-behavioural, psychological, and social features of individuals with lower-limb amputation in the presence of lower back pain. Methods and analysis This proposed protocol employs a prospective longitudinal study design that aims to explore the determinants of lower back pain amongst 30 adults with unilateral lower-limb amputation over 12-months. Biomechanical gait variables, trunk and lower-limb muscle activations, and objective pain measurements will be monitored every 3-months, and their relationship will be investigated. This information can be used to explore the characteristics of lower back pain and will inform future care management and rehabilitation processes. A data repository will be created and will be accessible through the University of Bath library website (https://library.bath.ac.uk/home). Ethics and dissemination This research has ethical approval from HRA ethics (REC reference: 23/EE/0090) agreed 28/04/23. The study results will be submitted for publication in a peer-reviewed clinical biomechanics journal, and disseminated through conference presentations.
