Factors Associated with Quality of Life Among Rural Women with HIV Disease

This cross-sectional study examined physical, psychological, and social factors associated with quality of life (QOL) among a sample of 399 rural women with HIV disease living in the Southeastern United States. Of the socio-demographic variables, age (p = .003), race (p < .0001), and time of HIV diagnosis (p = .03) were significantly associated with QOL. In bi-variate analysis, HIV symptoms (frequency and extent symptoms were bothersome), perceived stigma, internalized stigma, and depression were significantly and negatively associated with QOL whereas social support, problem-focused coping, perceived situational control, and healthy lifestyles were significantly and positively associated with QOL (all p < .0001). In adjusted analysis, HIV symptom frequency, depression, problem-focused coping, perceived situational control, perceived stigma, healthy lifestyles, and race remained significant predictors of QOL and explained 55% of the variance in QOL among the study participants (model F (7, 390) = 66.7; p < .0001). The study findings identify potential points of interventions to improve QOL among rural women with HIV disease.

Although healthy lifestyles are related to improved quality of life in the general population, little is known about the role of healthy lifestyles during HIV infection. The authors examined the relationships between health-promoting behaviors, risk behaviors, stress, and health-related quality of life (HRQOL) among 226 men with HIV infection who were attending three infectious disease clinics. As hypothesized, health-promoting behaviors were positively related and stress was negatively related with most of the HRQOL dimensions. Contrary to the hypothesis, tobacco use, recreational drug use, and unsafe sexual behaviors were not related to the HRQOL dimensions. Hazardous alcohol use was negatively associated with one HRQOL dimension--social functioning. The association of modifiable factors, such as health-promoting behaviors and stress, with HQROL offers opportunities for improving HIV-related health care. Relatively simple, straightforward changes in lifestyles such as eating well, remaining active, and avoiding stressful life events may result in improvements in HRQOL.

RESUMOIndivíduos que se submetem ao teste sorológico para dosagem de anticorpos contra o Vírus da Imunodeficiência Humana (anti-HIV ) podem ficar expostos a grande dose de estresse, nem sempre suportada pela estrutura psíquica, com o risco de ocorrer suicídio ou ato desordenado. Isto demonstra as implicações psíquicas pelas quais o indivíduo passa ao realizar exame, independente do resultado. A realização do exame exige uma série de diferentes cuidados e atenção para com esses indivíduos. É importante avaliar-se quanto esses indivíduos gostariam de saber, qual a possível reação frente a um resultado positivo. Por outro lado um resultado negativo também traz grande ansiedade havendo ainda os que não acreditam que o resultado seja negativo e com isso estão sendo poupados pela equipe de saúde.
