Abstract: SummaryUnderstanding the complex immunological consequences of red cell transfusion is essential if we are to use this valuable resource wisely and safely. The decision to transfuse red cells should be made after serious considerations of the associated risks and benefits. Immunological risks of transfusion include major incompatibility reactions and transfusion-related acute lung injury, while other immunological insults such as transfusion-related immunomodulation are relatively underappreciated. Red cell tr… Show more

“…3,7,8,17,18 Transfusion is a mortality risk factor in critically ill children 19 and may cause immune and non immune-mediated reactions including, more frequently: febrile nonhemolytic transfusion reaction, hemolytic reaction, alergies, volume overload and infections. 1,2,5,20 Microcirculatory changes can also occur, 5,21 as well as increased incidence of cancers. 22 Reactions may occur in 0.95% 23 to 3.8% of cases.…”

“…Irradiation of blood components prior to transfusion using gamma radiation has become a routine practice in many clinical institutions, and the main purpose for such a practice is to eliminates the risk of transfusion-associated graft versus host disease (TA-GVHD) [1][2][3][4]. The reason for irradiating blood instead of treating the patients with medications, such as immunoglobulins and corticosteroids, is the lack of medication efficacy.…”

“…Although our study does not clarify further the mechanism by which transfusions are detrimental, our study nonetheless adds to the body of evidence that perioperative transfusion has an adverse effect on short-term outcomes. It has long been recognized that autologous transfusion results in a state of relative immunosuppression, 8,16 which likely accounts for the increased risk for infectious complications. Paradoxically, transfusion also increases inflammation by inducing alloimmunization, which is the underlying mechanism for transfusion reactions, such as transfusion-related acute lung injury and multiorgan failure.…”
