Abstract: Marijuana abuse is rapidly growing and currently it is the most coimnon drug of abuse in the United States due to increased legalization for recreational and medicinal use. Delta 9-tetrahydrocannibol, the main psychoactive compound in marijuana, acts via the endocannabinoid system to elicit various cardiovascular physiological effects, and has been associated with many adverse cardiovascular effects such as acute coronary syndrome, arrhythmias, and sudden cardiac death that have previously been reported by our… Show more

“…This results in the generation of multiple ROS, which is a vasoconstrictive stimulus leading to smooth muscle constriction within coronary vessels [6]. Of note, not only are ROS a vasoactive stimulus but they also promote endothelial damage, starting the pathogenesis in the first place [4,6] Multiple cases of coronary slow and no flow phenomenon has been reported [10,11,12]. Coronary Slow Flow Phenomenon (CSFP) is an angiographic finding, where by there is a delay in distal vessel opacification in the absence of a significant occlusion and/or stenosis of the coronary vessel [13].…”

“…The authors hypothesized that marijuana-associated thrombosis is responsible for his recurrent occlusion. Other potential mechanisms include CB1R-mediated endothelial dysfunction and vasospasms leading to total arterial occlusion resulting in ischemia [69]. THC-induced endothelial dysfunction is caused by oxidative stress and reduced nitric oxide production.…”

“…Physicians should be aware of the varied cardiovascular adverse effects of marijuana such as myocardial infarction [9,25], cardiac arrhythmia [26] and myocarditis [27]. Abstinence is the key to prevent recurrent myocardial infarctions secondary to marijuana use [28]. The differential diagnosis for ST segment elevation related to marijuana use includes myocarditis [29] and coved ST-elevation of brugada pattern [30].…”
