Background and objectives In 2015, the ISBT Working Party on Blood Supply Management published the results of an international survey on blood product wastage in hospitals. The responses were predominantly from Europe and North America, and it was decided to form another survey concentrating on low-and middle-income countries (LMIC). Materials and methodsThe survey was designed by the authors and translated into an online survey tool by the ISBT office. A pilot survey was produced and completed by 3 volunteer contributors. Where necessary, amendments were made and the survey was sent by the ISBT to 33 participants from LMIC. The survey was further distributed by participants to colleagues in their or other countries. ResultsIn total, 31 completed surveys were received from 11 different countries. Not all respondents answered all questions meaning different questions may have been answered by a different combination of respondents. The demographics of hospital type, size and distance from the blood supplier varied considerably. Redblood-cell (RBC) wastage was estimated to be <5% (n=9 respondents), 5-10% (n=3) or 10-25% (n=5). The most important category of wastage was outdated RBCs in the blood bank or after issue. Wastage as a result of an RBC being issued to a patient and not completely transfused because the patient did not require the entire volume was <10% (n=18), 10-20% (n=3), 20-30% (n=1), 40-50% (n=1) and >50% (n=1).Conclusion Wastage rates varied considerably between countries, and innovative solutions will be required to reduce this problem.

Background: Blood and its components are very important for human life as there is no substitute for human blood. No major surgical procedure can be performed without the use of blood in any hospital or clinic. Ideally in a good set up, wastage of blood and blood products should never occur. The aim of this study was to assess the burden of blood wastage in our blood bank, reasons for the wastage, and to cut down blood wastage to a minimal amount by adopting new methods and techniques.Methods: A retrospective study was carried out in Department of Blood Bank of Pt. Jawaharlal Nehru Govt. Medical College, Chamba (H.P) over a time period of one year from 1st January 2017 to 31st December 2017.Results: In our study period, there were 35 discarded blood units due to various reasons forming 3.52% of total discard. Majority of units were discarded due to expiry (51.4%).Conclusions: Our study found expiry/outdated units to be most common reason for discard (51.4%). This kind of wastage may be reduced by better management of blood bag collection, storage and utilization. TTI was another significant reason for blood discard in our blood bank (14.28%). We can bring down the number of discard units by proper training and educating our blood bank staff. Strict adherence to donor selection criteria and proper past medical history should be obtained from blood donors.

Objective: To describe the reasons for the disposal of blood in the coordinating blood center of the State of Paraná and to estimate the financial costs resulting from potentially avoidable discards. Method: A descriptive, retrospective and documentary analysis, with data related to the period from 2010 to 2015 of a Brazilian coordinating blood center collected from a governmental database and analyzed by descriptive statistics. This study was approved by the Ethics Research Committee (CAEE 63074916.0.0000.5225). Results: 101,813 units were discarded, representing 22.3% of the total of 455,684 produced; plasma was the most discharged blood component. The main reason for discarding was lipemia (35.8%); the analysis showed that 56.9% of the disposals were considered potentially avoidable with an estimated paid value of approximately US$2 million. Conclusion: The expressive potential of avoidance of disposal of blood units and blood components highlights the importance of planning actions aiming at their best use, contributing to the reduction of amounts paid for these processes.
